摘 要:目的:通过分析研究内蒙古包头地区感染幽门螺旋杆菌(H.pylori)菌株的空泡毒素基因(vacuolating cytotoxin gene,VacA)亚型,探讨H.pylori在内蒙古地区的流行病学特点。方法:收集2018年6月至2019年6月在包头医学院第二附属医院就诊的308例不同胃、十二指肠相关疾病患者的H.pylori感染阳性患者活检标本,通过DNA分析,利用聚合酶链式反应技术扩增VacA基因亚型,从而对VacA基因型进行研究。结果:通过对308例H.pylori感染患者分析发现,VacA的总阳性率为57.14%,包头市区内VacA(+)52.31%,旗县区VacA(+)55.75%,市区与旗县区之间无统计学意义。VacA亚型阳性率sla为91.32%、slb为8.65%、s2为0.56%、ml为32.67%、m2a为89.54%、m2b为28.32%。vacA组合基因型(亚型嵌合体)以sla/m2a为主,占67.05%。sla、mlb型与相关疾病分布差异无统计学意义,而vacA的m2a型与疾病分布差异有统计学意义。结论:内蒙古包头地区幽门螺旋杆菌VacA基因分型以sla及m2a为主,其中m2a与疾病来源有关,这为内蒙古地区H.pylori感染的防控提供新的理论依据。Objective:To explore the epidemiological characteristics of Helicobacter pylori in Inner Mongolia through the analysis and research of H.pylori infection in Baotou area of Inner Mongolia.Methods:308 patients with H.pylori infection in the Second Affiliated Hospital of Baotou Medical College from June 2018 to June 2019 were collected.The vacA genotypes were amplified by polymerase chain reaction(PCR)through DNA analysis.Result:It was found through the analysis of 308 cases of H.pylori infection that the total positive rate of VacA was 57.14%,52.31%in Baotou City and 55.75%in country district,and there was no statistical significance between urban area and country district.The positive rate of VacA subtype was 91.32%in SLA,8.65%in SLB,0.56%in S2,32.67%in ml,89.54%in M2A and 28.32%in M2b.The main genotype(subtype chimerism)of VacA was SLA/M2A,accounting for 67.05%.There was no significant difference between the distribution of SLA and MLB and the distribution of related diseases,but there was significant difference between M2A(a type of VacA)and disease distribution.Conclusion:The main VacA genotypes of Helicobacter pylori in Baotou area of Inner Mongolia are SLA and M2A,among which M2A is related to the source of the disease,which provides a new theoretical basis for the prevention and control of H.pylori infection in Inner Mongolia.
