摘 要:目的了解三年制临床实习生工作倦怠、转行意愿的现状,分析其相关性及影响因素,为医学院校临床教育部门实施有效管理提供参考。方法采用一般情况调查表、职业选择动机与转行意愿调查表及职业倦怠量表,对661名三年制临床实习生开展问卷调查。分析其工作倦怠、转行意愿的检出率,并探讨两者的相关性。采用多元线性回归分析转行意愿的影响因素。结果三年制医学生情感衰竭、去人性化及个人成就感维度得分分别为(16.03±6.67)分、(3.77±2.20)分、(39.78±8.62)分;转行意愿检出率为36.5%。情感衰竭、去人性化与转行意愿呈正相关(r=0.294,P<0.05;r=0.340,P<0.05),个人成就感与转行意愿呈负相关(r=-0.326,P<0.05);生源地、职业选择动机与转行意愿呈正相关(r=0.069,P<0.05;r=0.248,P<0.05)。多元线性回归分析显示,热爱医学、实现自我价值、认为工作稳定、满足家人意愿对转行意愿有影响(P<0.05)。结论三年制医学生实习工作倦怠与转行意愿情况并不乐观,其影响因素较为复杂。相关部门及实习医院应采取针对性措施,缓解学生的工作倦怠,从而减少医学生的转行意愿。Objective To investigate the current situation of job burnout and turnover intention of three-year clinical interns and analyze their correlation and influencing factors,so as to provide basis for the three-year clinical intern management department to formulate intervention measures.Methods 661 three-year clinical interns were investigated by general situation questionnaire,career choice motivation and turnover intention questionnaire and job burnout scale.Analysis was done to analyze the detection rate of job burnout and turnover intention,and to explore the correlation between them and the influence of career choice motivation on turnover intention.Results The scores of emotional exhaustion,dehumanization and personal achievement of three-year medical students were 16.03±6.67,3.77±2.20,and 39.78±8.62 respectively.The detection rate of transfer intention was 36.5%.There was a positive correlation between emotional exhaustion,depersonalization and turnover intention(r=0.294,P<0.05;r=0.340,P<0.05).A negative correlation was found between personal achievement and turnover intention(r=-0.326,P<0.05),and a positive correlation existed between the motivation of origin and career choice and turnover intention(r=0.069,P<0.05;r=0.248,P<0.05).The results of multiple linear regression analysis showed that the factors of loving medicine,realizing self-worth,working stability and satisfying family's willingness had an effect on the turno-ver intention(P<0.05).Conclusion The situation of job burnout and turnover intention of three-year medical students is not optimistic.The influencing factors are complicated.Relevant education departments and practice hospitals should take measures to alleviate the job burnout of students,so as to reduce the turnover intention of medical students.
