聚焦乳腺癌患者“心理痛苦”视角下的人文关怀模式

摘　　要：目的通过量化与质性研究,聚焦乳腺癌患者因心理痛苦产生根源及转归理论模型的构建及尝试建立人文关怀模式。方法对558例乳腺癌患者筛查出心理痛苦患者,通过参与式观察、深度访谈,撰写备忘录等方法进行归类综合,以聚焦图表的形式进行分类并整理分析。结果心理痛苦的核心编码为失去自信,乳腺癌心理痛苦信念行为的转变有自我认知的调整,转折事件中通过人文关怀中专业支持、关爱力量使乳腺癌患者感受到支持,可促进其自信心的恢复。结论通过重视个体化的人文关怀需求,关爱中心成立、多学科会诊、对症状进行管理、关注情感,帮助患者走出心理局限、利用网络平台跟踪服务等措施进行专业支持与人文关怀。重视乳腺癌患者心理痛苦体验,人文关爱与专业支持共同构建形成人文关怀模式。Objective:Through quantitative and qualitative research,focusing on the causes of psychological pain in breast cancer patients and the construction of the theoretical model of the outcome and trying to establish a humanistic care model.Methods:558 cases of breast cancer patients were screened out with psychological pain.They were classified and integrated through participatory observation,in-depth interview,writing memos and other methods,and were classified and sorted out in the form of focus chart.Results:The core code of psychological pain was loss of self-confidence.The change of psychological pain belief behavior of breast cancer had adjustment of self-cognition,the professional support and caring power in humanistic care,which could promote the recovery of breast cancer patients'self-confidence.Conclusion:Through paying attention to the needs of individualized humanistic care,the establishment of care center,multi-disciplinary consultation,symptom management,emotional concern,which could help to break out of psychological limitations,and use network platform tracking services and other measures for professional support and humanistic care.Pay attention to the psychological pain experience of breast cancer patients humanistic care and professional support to build a humanistic care model.
