FSCN1蛋白在胃癌组织中的表达及临床意义

摘　　要：目的探讨Fascin-1(FSCN1)蛋白在胃癌组织中的表达及其临床意义,为临床诊治胃癌提供参考依据。方法选择均经病理诊断确诊的胃癌患者102例,男性68例,女性34例;年龄40~73岁,平均57.8岁;Ⅰ+Ⅱ期60例,Ⅲ+Ⅳ期42例;高分化15例,中分化21例,低分化为66例;淋巴结转移62例。20例癌旁组织(距离癌组织>5 cm)作为对照组。应用免疫组织化学法检测胃癌组织(胃癌组)、癌旁组织(20例,癌旁组织组)中FSCN1的表达阳性率。分析癌组织中FSCN1表达与胃癌转移、分期及预后的关系。结果胃癌组织FSCN1阳性表达率[55.9%(57/102)]高于癌旁组织[15.00%(3/20)],差异有显著统计学意义(χ^(2)=17.763,P=0.000)。胃癌组织中FSCN1表达与患者分化程度、淋巴结转移和TNM分期有显著相关性(均P<0.05)。FSCN1阳性组中位生存时间低于FSCN1阴性组[(28.00±2.20)个月vs(46.00±3.05)个月)(P=0.000);FSCN1阳性组5年生存率[24.7%(14/57)]低于FSCN1阴性组[55.6%(25/45)](χ^(2)=14.384,P=0.000)。多因素Cox回归分析显示,TNM分期(Ⅲ+Ⅳ)、FSCN1高表达水平是胃癌患者随访5年期间预后的危险因素(P<0.05)。结论FSCN1在胃癌组织中高表达,与肿瘤的进展、预后有关。Objective To investigate expressions of Fascin-1(FSCN1)in gastric cancer tissue and its significance,and provide basis for clinical diagnosis and treatment.Methods A total of 102 patients with gastric cancer diagnosed by pathological diagnosis were enrolled,which included 68 males and 34 females,aged 40-73 years old with mean age of 57.8 years old;60 cases of stageⅠ+Ⅱand 42 of stageⅢ+Ⅳ;15 cases of high differentiation,21 of moderate differentiation and 66 of low differentiation,and 62 cases of lymph node metastasis.Twenty cases of paracancerous tissue(>5 cm from cancer tissue)were set as control group.The expression rate of FSCN1 in gastric cancer tissues(gastric cancer group,n=102)and adjacent normal gastric tissues(paracancerous tissue group,n=20)were detected by immunohistochemical method.The relationship between expressions of FSCN1 and cancer metastasis,different staging and prognosis were analyzed.Results The positive expression rate of FSCN1 in gastric cancer tissue[55.9%(57/102)]was obviously higher than that in normal tissues[15.00%(3/20)],and the difference was statistically significant(χ^(2)=17.763,P=0.000).The expressions of FSCN1 was significant correlated with tumor differentiation,lymph node metastasis and TNM stage(all P<0.05).The median survival time of FSCN1 positive group was lower than that of FSCN1 negative group[(28.00±2.20)months vs(46.00±3.05)months](P=0.000).Five-year survival rate of FSCN1 positive group[24.7%(14/57)]was lower than that of FSCN1 negative group[55.6%(25/45)](χ^(2)=14.384,P=0.000).The multivariate Cox regression analysis showed that TNM stage(Ⅲ+Ⅳ),FSCN1 high expression level were risk factors for prognosis in patients with gastric cancer during 5 years follow-up(P<0.05).Conclusion It is demonstrated that FSCN1 is highly expressed in gastric cancer tissues,which correlates with tumor progression and prognosis.
