宫颈鳞癌组织中CEMIP、β-catenin、Cyclin D1的表达变化及其意义

摘  要:目的观察宫颈鳞癌组织中细胞迁移诱导的透明质酸结合蛋白(CEMIP)、β-连环蛋白(β-catenin)、细胞周期蛋白D1(Cyclin D1)的表达变化,并探讨其意义。方法选取宫颈鳞癌组织96例份、Ⅱ~Ⅲ期宫颈上皮内瘤变(CIN)组织30例份、Ⅰ期CIN组织30例份、慢性宫颈炎组织30例份,采用免疫组化法检测上述组织CEMIP、β-catenin、Cyclin D1,分析三指标间及其与宫颈鳞癌临床病理参数的关系,Kaplan-Meier生存分析法分析三指标阳性表达与宫颈鳞癌患者生存期的关系,Cox回归分析法分析宫颈鳞癌患者预后的危险因素。结果与宫颈鳞癌比较,Ⅱ~Ⅲ期CINβ-catenin、Cyclin D1阳性率低,Ⅰ期CIN及慢性宫颈炎CEMIP、β-catenin、Cyclin D1阳性率低(P均<0.05);与Ⅱ~Ⅲ期CIN比较,慢性宫颈炎CEMIP、β-catenin、Cyclin D1阳性率低(P均<0.05)。CEMIP、Cyclin D1阳性表达与宫颈鳞癌FIGO分期、分化程度、浸润深度、淋巴结转移有关,β-catenin阳性表达与宫颈鳞癌FIGO分期、分化程度、淋巴结转移有关(P均<0.05)。宫颈鳞癌组织中CEMIP与β-catenin、Cyclin D1表达呈正相关(r分别为0.301、0.453,P均<0.05),β-catenin与Cyclin D1表达呈正相关(r为0.495,P<0.05)。与CEMIP、β-catenin、Cyclin D1阴性者比较,阳性者5年生存率低(P均<0.05)。FIGO分期、分化程度、浸润深度、SCC值、淋巴结转移及CEMIP、β-catenin、Cyclin D1阳性表达是宫颈鳞癌患者预后影响因素,淋巴结转移及CEMIP、β-catenin、Cyclin D1阳性表达是宫颈鳞癌患者预后危险因素。结论宫颈鳞癌组织中CEMIP、β-catenin、Cyclin D1表达升高,其阳性表达与患者不良预后密切相关,上述三指标可作为宫颈鳞癌病变进展和预后的有效预测指标及潜在的治疗靶点。Objective To observe the expression changes of cell migration-inducing and hyaluronan-binding protein(CEMIP),β-catenin,and Cyclin D1 in the cervical squamous cell carcinoma tissues and to explore their significance.Methods Immunohistochemical method was used to detect the expression levels of CEMIP,β-catenin,and Cyclin D1 in 96 cases of cervical squamous cell carcinoma tissues,30 cases of cervical intraepithelial neoplasias(CIN)Ⅱ-Ⅲ,30 cases of CIN I,and 30 cases of chronic cervicitis,and we analyzed the relationship between the three indicators and the clinicopathological characteristics of cervical squamous cell carcinoma patients;Spearman rank correlation analysis was used to analyze the correlation among the three factors;Kaplan-Meier survival analysis was used to analyze the relationship between the positive expression of three indicators and survival time of patients with cervical squamous cell carcinoma;Cox proportional hazard model was used to analyze the risk factors affecting the prognosis of patients.Results Compared with patients with cervical squamous cell carcinoma,patients with CINⅡ-Ⅲhad lower positive rates ofβ-catenin and Cyclin D1,patients with CIN I and chronic cervicitis had lower positive rates of CEMIP,β-catenin,and Cyclin D1(all P<0.05);compared with patients with CINⅡ-Ⅲ,patients with chronic cervicitis had lower positive rates of CEMIP,β-catenin,and Cyclin D1(all P<0.05).The positive expression of CEMIP and Cyclin D1 were significantly related to FIGO staging,degree of differentiation,depth of invasion,and lymph node metastasis of cervical squamous cell carcinoma(all P<0.05),and the positive expression ofβ-catenin was significantly related to FIGO staging,degree of differentiation,and lymph node metastasis of cervical squamous cell carcinoma(all P<0.05).Spearman correlation analysis showed that,in the cervical squamous cell carcinoma tissues,CEMIP was positively correlated withβ-catenin and Cyclin D1(r=0.301 and 0.453,respectively,both P<0.05)andβ-catenin was positively
