摘  要:目的探讨CT灌注成像评价家犬急性缺血性脑梗死模型的价值。方法12只健康雄性家犬按照随机数字表法分为模型组6只与对照组6只。模型组采用介入技术方法建立急性缺血性脑梗死模型,对照组不建立模型。造模后1周对家犬进行CT灌注成像评价,测定感兴趣区的脑血流量(CBF)、脑血容量(CBV)、局部灌注达到峰值时间(TTP);并测量感兴趣区的CT值。在造模前、造模后1周在CT影像图像对大脑三径线进行测量,计算大脑体积。于光学显微镜下观察犬大脑组织病理情况。结果模型组有2只犬在模型制作过程中死亡,对照组无犬只死亡。模型组CBV、CBF值为(1.64±0.26)mL·100g^(-1)、(22.10±2.22)mL·100g^(-1)·min-1,显著低于对照组[(2.89±0.15)mL·100g^(-1)、(37.20±2.11)mL·100g^(-1)·min-1],TTP值(17.49±0.77)s显著高于对照组[(14.98±1.49)s],差异均有统计学意义(P<0.05)。模型组常规CT扫描与增强CT扫描的大脑CT值为(46.75±3.71)、(87.02±12.09)Hu,均显著低于对照组[(119.78±14.56)、(522.22±17.88)Hu],差异均有统计学意义(P<0.05)。造模后1周模型组的大脑体积(11.49±1.85)mL高于造模前[(9.22±0.55)mL],也高于对照组[(9.10±0.77)mL],差异有统计学意义(P<0.05),对照组造模前后比较,差异无统计学意义(P>0.05)。与对照组比较,模型组的梗死灶和正常区之间有明显的分界,肉眼下即可见地图样不规则梗死区并伴随有组织水肿,光镜下可见神经细胞和胶质细胞显著减少,部分区域见中性粒细胞浸润。结论CT灌注成像可反映急性缺血性脑梗死家犬的血流动力学与形态结构变化情况,从而可快速、有效、无创地评价家犬的病情状况。Objective To investigate the values of CT perfusion imaging in evaluating the model of acute ischemic cerebral infarction in dogs.Methods Twelve cases of healthy male domestic dogs were divided into 6 cases in the model group and 6 cases in the control group accorded to the random number table method.The model group were used interventional techniques to establish acute ischemic cerebral infarction model,and the control group were not established model.The dogs were evaluated by CT perfusion imaging one week after the model.The cerebral blood flow(CBF),cerebral blood volume(CBV),and time to peak local perfusion(TTP)in the area of interest were measured;and the CT value of the area of interest was measured.The three-dimensional lines of the brain were measured on the CT image before and 1 week after the modeling,and the brain volume was calculated.The pathological condition of canine brain tissue was observed under an optical microscope.Results Two dogs in the model group were died during the model making process,and no dogs in the control group were died.The CBV and CBF values of the model group were(1.64±0.26)mL·100g^(-1),(22.10±2.22)mL·100g^(-1)·min-1,which were significantly lower than those of the control group[(2.89±0.15)mL·100g^(-1),(37.20±2.11)mL·100g^(-1)·min-1],the TTP value(17.49±0.77)s was significantly higher than that of the control group[(14.98±1.49)s],the differences were statistically significant(P<0.05).The brain CT values of conventional CT scan and enhanced CT scan in the model group were(46.75±3.71),(87.02±12.09)Hu,which were significantly lower than those in the control group[(119.78±14.56),(522.22±17.88)Hu],the differences were statistically significant(P<0.05).One week after modeling,the brain volume of the model group(11.49±1.85)mL was higher than that before modeling[(9.22±0.55)mL]and also higher than that of the control group[(9.10±0.77)mL],the difference was statistically significant(P<0.05).There were no significant difference compared between the control group befor
