四种血清肿瘤标志物在肺癌诊断中的应用

摘　　要：目的探讨血清CEA、CYFRA21-1、NSE和SCC检测在肺癌诊断中的应用价值。方法选择2019年7—12月安徽医科大学第一附属医院就诊的肺癌患者45例、肺部良性疾病患者46例与健康体检者30例为研究对象,分为肺癌组、良性肺病组及对照组,采用电化学发光法检测血清CEA、CYFRA21-1、NSE和SCC浓度。结果肺癌组CEA、CYFRA21-1、NSE和SCC血清水平中位数分别为14.25 ng/mL、11.88 ng/mL、28.37 ng/mL、2.57 ng/mL,高于正常对照组的2.24 ng/mL、2.35 ng/mL、11.33 ng/mL、0.85 ng/mL,差异均有统计学意义(P<0.05);在单项检测中,CYFRA21-1灵敏度(84.44%)高于CEA、NSE和SCC(46.67%,37.78%,35.56%),但CYFRA21-1特异度(68.42%)低于CEA、NSE和SCC(86.84%,86.84%,85.53%);CEA、CYFRA21-1、NSE和SCC并联检测的灵敏度为93.33%,串联检测的特异度为98.68%。结论CEA、CYFRA21-1、NSE和SCC可作为肺癌诊断的血清学指标,联合检测能弥补单项血清标志物的不足,对提高肺癌的诊断具有重要的临床价值。Objective To research the application value of serum CEA,CYFRA21-1,NSE and SCC in the diagnosis of lung cancer.Methods Serum CEA,CYFRA21-1,NSE and SCC concentrations of 45 cases of patients with lung cancer,46 cases of patients with benign lung diseases and 30 cases of healthy check from July to December 2019 were divided into lung cancer group,benign lung diseases group and control group and detected by electrochemiluminescence.Results The median of levels of serum CEA,CYFRA21-1,NSE and SCC in the lung cancer group were 14.25 ng/mL,11.88 ng/mL,28.37 ng/mL and 2.57 ng/mL,respectively,which were significantly different with that in the control group(2.24 ng/mL,2.35 ng/mL,11.33 ng/mL and 0.85 ng/mL,respectively),with statistically significant differences(P<0.05).In the single test,the sensitivity of CYFRA21-1 was 84.44%,which was higher than those of CEA,NSE and SCC(46.67%,37.78%and 35.56%),but the specificity of CYFRA21-1 was 68.42%,which was lower than those CEA,NSE and SCC(86.84%,86.84%and 85.53%).The sensitivity of parallel detection of CEA,CYFRA21-1,NSE and SCC was 93.33%,and specificity of serial detection was 98.68%.Conclusion CEA,CYFRA21-1,NSE and SCC can be applied as serological indexes in the diagnosis of lung cancer.Combined detection can make up for the deficiency of single serum markers,which has important clinical value for improving the diagnosis of lung cancer.
