摘 要:目的探讨单孔腹腔镜下密闭式排烟装置的临床应用效果。方法选取2019年4—12月在肇庆市第二人民医院行单孔腹腔镜下手术的60例患者作为研究对象。按照随机数字表法将患者分为对照组和观察组,每组各30例。对照组患者采用传统单孔套件排气口排烟,观察组患者采用自制密闭式排烟装置排烟。比较两组患者的手术时间、术中出血量、术者暂停手术频率、暂停手术耗时、消耗CO2量、尘埃粒子计数、术者满意度评分及两组患者的术后近期并发症发生率。结果观察组患者的手术时间、术中出血量、术者暂停手术频率、暂停手术耗时均小于对照组,消耗CO2量多于对照组,差异有统计学意义(P<0.05)。观察组患者术中0.3、0.5、1.0、3.0、5.0μm尘埃粒子计数均少于对照组,差异有统计学意义(P<0.05)。观察组术者满意度评分低于对照组,差异有统计学意义(P<0.05)。两组患者术后近期并发症发生率比较,差异无统计学意义(P>0.05)。结论自制单孔腹腔镜下密闭式排烟装置可有效排出单孔腹腔镜下手术患者术中产生的烟雾,利于手术操作,减少手术室空气污染,且具有较好的安全性。Objective To explore the clinical application effect of the closed smoke exhaust device under the single port laparoscope.Methods A total of 60 patients who were treated with single-port laparoscopic surgery in the Second People′s Hospital of Zhaoqing from April to December 2019 were selected as the research objects.According to the random number table method,the patients were divided into control group and observation group,30 cases in each group.The control group was treated with traditional single port smoke exhaust set,while the observation group was treated with self-made closed smoke exhaust device.The operative time,intraoperative blood loss,frequency of operation suspension,duration of operation suspension,amount of CO2 consumption,dust particle count,satisfaction score of the operator and the incidence of recent postoperative complications were compared between the two groups.Results The operative time,intraoperative blood loss,frequency of operation suspension,and duration of operation suspension of the operator in the observation group were all lower than those in the control group,and the amount of CO2 consumption was higher than that in the control group,with statistically significant differences(P<0.05).The number of dust particles of 0.3,0.5,1.0,3.0,5.0μm in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).The satisfaction score of the operator in the observation group was lower than that in the control group,the difference was statistically significant(P<0.05).There was no statistically significant difference in the incidence of recent postoperative complications between the two groups(P>0.05).Conclusion The self-made closed smoke exhaust device under single port laparoscope can effectively discharge the smoke generated in single-port laparoscopic surgery,which is conducive to the operation,and can reduce air pollution in the operating room,with good safety.
