超声引导下经皮射频消融与腹腔镜手术治疗原发性肝癌的疗效及安全性比较

摘　　要：目的比较超声引导下经皮射频消融与腹腔镜手术治疗原发性肝癌的疗效及安全性。方法根据治疗方式将80例原发性肝癌患者分为消融组(n=42)和腹腔镜组(n=38),其中消融组患者接受超声引导下经皮射频消融治疗,腹腔镜组患者接受腹腔镜肝切除术治疗。比较两组患者的一般手术指标、肝功能指标、根治及复发情况、并发症发生情况及术后3年生存情况。结果消融组患者手术时间、手术住院时间均明显短于腹腔镜组患者,术中出血量、医疗费用均明显低于腹腔镜组患者,差异均有统计学意义(P﹤0.01)。两组患者术后肝功能指标、根治率和复发率、术后1~3年生存率比较,差异均无统计学意义(P﹥0.05)。消融组患者并发症总发生率为7.14%,低于腹腔镜组患者的26.32%,差异有统计学意义(P﹤0.05)。结论超声引导下经皮射频消融治疗原发性肝癌的疗效与腹腔镜手术的疗效相似,但射频消融术更加微创,有助于患者术后恢复,还可节省医疗费用。Objective To compare the efficacy and safety of ultrasound-guided percutaneous radiofrequency ablation and laparoscopic surgery in primary liver cancer.Method 80 patients with primary liver cancer were divided into ablation group(n=42,treated with ultrasound-guided percutaneous radiofrequency ablation)and laparoscopic group(n=38,treated with laparoscopic hepatectomy).The general surgical indexes,liver function,radical cure and recurrence,complications and 3-year survival of the two groups were compared.Result The operation time and hospital stay of the patients in the ablation group were significantly shorter than those in the laparoscopic group,the blood loss and medical expenses during the operation were significantly lower than those in the laparoscopic group.The differences were statistically significant(P<0.01).There was no statistically significant difference between the two groups of patients in terms of postoperative liver function indexes,radical cure rate and recurrence rate,and survival rate of 1 to 3 years after surgery(P>0.05).The total incidence of complications in the ablation group was 7.14%,which was lower than 26.32%in the laparoscopic group,and the difference was statistically significant(P<0.05).Conclusion The efficacy of ultrasound-guided percutaneous radiofrequency ablation for the treatment of primary liver cancer is similar to that of laparoscopic surgery,but radiofrequency ablation is more minimally invasive,helps patients recover after surgery,and saves medical costs.
