摘　　要：目的本文采用系统评价的方法分析国内小剂量肝素联合硫酸镁治疗孕妇子宫肌瘤红色变性的效果,为进一步探讨临床合理用药提供循证医学证据。方法针对国内治疗方案,首先遵循Cochrane协作系统评价的标准,检索国内中国医学生物文献数据库、中国知网和万方数据库,查阅与肝素医治妊娠合并子宫肌瘤红色变性有关的文献;然后按照事先设定好的方案进行文献遴选并对其质量评价;最后应用Rev Man 5.3行数据分析。结果经挑选,国内共有相关关文献11篇累计523例患者,实验组总有效率为244/263(92.78%),对照组总有效率为166/260(63.85%)。统计分析提示前者的临床效果相较后者更好[OR值95%CI为7.31(4.29,12.45),P<0.05]。结论肝素对我国妊娠期子宫肌瘤红色变性的患者治疗效果较好,能明显缓解患者疼痛及有效地抑制宫缩降低早产及流产的风险,对母儿取得良好结局有益。Objective To analyze the clinical effect of domestic low-dose heparin combined with magnesium sulfate in the treatment of red degeneration of uterine fibroids during pregnancy,for providing medical evidence for further plans,first of all,according to the standards of systematic evaluation by disscussion of rational clinical use of drugs.Methods For domestic treatment Cochrane collaboration,the Chinese medical and biological literature database,China knowledge network and wanfang database were searched,and the literatures related to heparin treatment of pregnancy complicated with uterine fibroid red degeneration were reviewed.Then select the appropriate literature according to the pre-set scheme and evaluate its quality.Finally,Rev Man 5.3 was applied for data analysis.Results After selection,a total of 523 patients were included in 11 domestic literatures.The total effective rate of the experimental group was 244/263(92.78%),and that of the control group was 166/260(63.85%).Statistical analysis suggested that the clinical effect of the experimental group was better than that of the control group[OR=7.31,95%CI(4.29,12.45),P<0.05].Conclusions Heparin has a good therapeutic effect on the patients with uterine fibroid degeneration during pregnancy in China,which can significantly relieve the patients'pain and inhibit contractions,reduce the risk of premature delivery and miscarriage,and thus benefit the pregnancy outcome.
