摘　　要：目的分析整合医学模式下远程视频会诊服务发展现状,展望远程视频会诊医疗服务平台可提供的其他医疗服务。方法选取2015年11月至2019年12月河南省人民医院18364例远程会诊病例,描述申请会诊医院辐射距离、会诊科室、会诊类型等特点,分析会诊目的,调查申请医生对远程会诊的满意度。结果远程会诊医院间距离为>100~200、>200~300、>300~400、>400~500、>500~600、>600~700 km的会诊病例依次占11.30%(2076/18364)、21.24%(3901/18364)、13.20%(2424/18364)、24.40%(4480/18364)、12.12%(2225/18364)、13.20%(2424/18364)。申请会诊数量排名前三的科室为神经内科、儿科和呼吸内科,分别占13.51%(2481/18364)、7.97%(1463/18364)、7.58%(1392/18364)。预约会诊、点名会诊、紧急会诊、多学科会诊分别占85.60%(15720/18364)、9.10%(1671/18364)、2.70%(496/18364)、2.60%(477/18364)。预约会诊、点名会诊、紧急会诊、多学科会诊的平均完成时间分别为(20.2±9.8)、(38.4±5.2)、(1.4±0.4)、(40.1±10.3)h。18364例患者的会诊目的中,诊断疾病2930例(16.00%),指导治疗6833例(37.21%),诊断并指导治疗8601例(46.84%),最终16583例(90.30%)患者达成会诊目的。99.20%的医生愿意继续进行远程会诊。结论加强在距离200~500 km医院的远程视频会诊的宣传力度,合理分配会诊数量较多的科室医疗资源,为更多患者提供及时、便捷、优质的远程医疗服务。Objective To analyze the current status of remote video consultation service under the holistic integrative medicine,and to look forward to other medical services that could be provided by the remote video consultation platform.Methods A total of 18364 cases of remote video consultations from November 2015 to December 2019 were selected from Henan Provincial People’s Hospital.The characteristics of radiation distances,consultation departments and consultation types were described.The purpose of consultation was analyzed and the satisfaction degree of doctors applying for remote consultation was investigated.Results The consultation cases whose distance between hospitals of remote consultation and Henan Provincial People’s Hospital was>100~200,>200~300,>300~400,>400~500,>500~600,>600~700 km accounted for 11.30%(2076/18364),21.24%(3901/18364),13.20%(2424/18364),24.40%(4480/18364),12.12%(2225/18364),13.20%(2424/18364),respectively.The top three departments for consultation were Neurology Department,Pediatrics Department and Respiratory Department,accounting for 13.51%(2481/18364),7.97%(1463/18364)and 7.58%(1392/18364),respectively.Appoint consultation,roll call consultation,emergency consultation and multidisciplinary consultation accounted for 85.60%(15720/18364),9.10%(1671/18364),2.70%(496/18364)and 2.60%(477/18364),respectively.The average time of appointment consultation,roll call consultation,emergency consultation and multidisciplinary consultation were(20.2±9.8),(38.4±5.2),(1.4±0.4)and(40.1±10.3)hours,respectively.Among the 18364 patients,2930 cases(16.00%)were diagnosed for disease,and 6833 cases(37.21%)were guided for treatment,and 8601 cases(46.84%)were diagnosed and guided for treatment.A total of 16583 cases(90.30%)finally achieved the purpose of consultation.99.20%of doctors were willing to continue remote consultation.Conclusion To strengthen the publicity of remote video consultation in the hospital with a distance of 200~500 km,rationally allocate medical resources of departments with a large
