摘 要:目的建立中华小型猪急性心肌梗死(AMI)后心肌炎症模型,并采用病理分析和^(18)F-FDG PET/CT方法进行验证。材料与方法中华小型猪15只,按简单随机抽样法分为正常对照组、假手术组和AMI组,每组5只。AMI组使用经皮冠状动脉球囊结合凝胶海绵联合栓塞法阻塞冠状动脉前降支建立AMI炎症模型;假手术组除不使用栓子进行血流阻断外,其余操作均与AMI组一致;正常对照组不做特殊处理。15只猪均行^(18)F-FDG PET/CT显像,显像前使用联合方案抑制心肌的生理性摄取。在左心室前壁(梗死区)勾画感兴趣区,测量最大标准化摄取值(SUVmax)。采用免疫组化染色法观察各组CD68+巨噬细胞和葡萄糖转运体(GLUTs)的表达。结果AMI组18F-FDG摄取SUVmax为4.32±0.58,明显高于正常对照组的1.10±0.10及假手术组的1.08±0.08(P<0.001)。免疫组化染色显示AMI组梗死区大量CD68+巨噬细胞浸润,GLUT-1和GLUT-3表达强度明显高于正常对照组和假手术组。AMI组、正常对照组和假手术组中GLUT-4均仅有微弱表达。结论本研究成功建立了中华小型猪AMI后炎症反应模型,18F-FDG PET/CT可以无创地评价AMI后的炎症反应。Purpose To establish an acute myocardial infarction(AMI)inflammation model in Chinese miniature swine,and evaluate the feasibility of this model through pathological analysis and 18F-FDG PET/CT scanning.Materials and Methods Fifteen Chinese miniature swine were randomly divided into normal control group,sham-operation group and AMI group,with five swine in each group.In AMI group,AMI inflammation model was induced using percutaneous coronary balloon embolization combined with geofoam embolization to occlude the left anterior descending coronary artery.The sham-operation group was consistent with the AMI group except no embolus was used for blood flow occlusion.The normal-control group had no special treatment.18F-FDG PET/CT scanning was performed thereafter,with a comprehensive strategy to suppress the physiological uptake of cardiomyocytes.The region of interest was delineated on the anterior wall of the left ventricle(infarct area)and the maximum standardized uptake value(SUVmax)was measured.Immunostaining was employed to investigate CD68+macrophage and glucose transporters(GLUTs).Results 18F-FDG uptake in AMI group was significantly higher than that in normal-control group and sham-operation group(SUVmax:4.32±0.58 vs.1.10±0.10 vs.1.08±0.08,P<0.001).Immunostaining demonstrated robust recruitment of CD68+macrophages to the infarct area of AMI group,the expression intensity of GLUT-1 and GLUT-4 was obviously higher than that of normal-control and sham-operation swine.The expression of GLUT-4 was weak in both AMI,normal-control group and sham-operation group.Conclusion The Chinese miniature swine model is suitable for assessing post-AMI inflammation and for pre-clinical study,PET/CT can noninvasively evaluate post-AMI inflammation.
