摘  要:目的探讨嗜酸性粒细胞趋化因子(Eotaxin)和白三烯B4(LTB4)对直肠癌围术期输血患者发生不良反应的影响。方法研究对象为2016年6月至2019年6月确诊直肠癌并入住我院的患者,均采取输血治疗,选择发生输血反应的66例纳入观察组,同期选择未发生输血反应60例纳入对照组,总共126例,所有患者的血清Eotaxin及LTB4均采用酶联免疫吸附测定(ELISA)法进行检测,两组患者单因素分析影响输血反应的相关因素,多因素Logistic回归确认关键危险因素;绘制输血反应发生的ROC曲线,考察血清Eotaxin及LTB4对输血反应的预测价值,并探究血清Eotaxin及LTB4联合诊断输血反应发生的敏感度与特异性。结果两组患者基础数据中,观察组患者的既往输血史、过敏史、发血-输血间隔及输血次数4个参数显著高于对照组(P<0.05);输血前,两组患者的血清Eotaxin和LTB4处于同等表达水平(P>0.05),输血后,观察组的Eotaxin及LTB4较对照组及输血前均明显增高(P<0.05),而对照组输血后则保持与输血前相当水平,未产生显著变化(P>0.05)。Logistic回归分析可知,有既往输血史(OR=2.336,95%CI:1.059~5.153)、过敏史(OR=3.880,95%CI:1.475~10.208)、发血-输血间隔≥0.5h(OR=2.745,95%CI:1.113~6.769)、输血超过2次(OR=2.672,95%CI:1.078~6.620)、血清Eotaxin(OR=3.185,95%CI:1.253~8.098)及血清LTB4(OR=3.594,95%CI:1.457~8.863)是影响直肠癌患者输血反应发生的独立危险因素。ROC曲线结果可知,以57.6μg/L作为预测的临界值,血清Eotaxin的ROC曲线下面积为0.813,灵敏度为76.1%,特异性为71.5%;以16.8 pg/L作为预测的临界值,血清LTB4的ROC曲线下面积为0.865,预测的灵敏度为80.8%,特异性为82.5%。结论血清Eotaxin及LTB4与患者输血反应的发生关系密切,可作为输血反应发生的潜在标志物。Objective To investigate the effects of Eosinophil chemotactic factor(Eotaxin)and Leukotriene B4(LTB4)on adverse reactions in perioperative blood transfusion patients with rectal cancer.Methods The subjects for the current study included the patients with rectal cancer admitted to our hospital from June,2016 to June,2019,all of whom were treated with blood transfusion,while 66 patients with transfusion reaction were selected into the observation group,and 60 patients without transfusion reaction were selected into the control group.In total,there were 126 cases and all patients’serum Eotaxin and LTB4 levels were detected by enzyme linked immunosorbent assay(ELISA).The potential factors influencing blood transfusion reaction were analyzed by single factor analysis,and the key risk factors were confirmed by multivariate logistic regression.The ROC curve of blood transfusion reaction was drawn to investigate the predictive value of serum Eotaxin and LTB4 on blood transfusion reaction.The sensitivity and specificity of joint diagnosis of blood transfusion reaction by serum Eotaxin and LTB4 were also evaluated.Results In the basic data of the two groups,the past blood transfusion history,allergy history,blood transfusion interval and blood transfusion times of the observation group were significantly higher than those of the control group(P<0.05).Before blood transfusion,the serum Eotaxin and LTB4 of the two groups were at the same level(P>0.05).After blood transfusion,the results were as following:the Eotaxin and LTB4 in the observation group were significantly higher than those in the control group and before blood transfusion(P<0.05),while the levels of Eotaxin and LTB4 in the control group remained at the same level as before blood transfusion,without significant changes(P>0.05).Logistic regression analysis showed that previous blood transfusion history(OR=2.336,95%CI:1.059-5.153),allergic history(OR=3.880,95%CI:1.475-10.208),blood transfusion interval≥0.5 h(OR=2.745,95%CI:1.113-6.769),blood transfusion more t
