探讨高频电波刀电圈切除术治疗宫颈上皮内瘤变伴高危型人乳头瘤病毒感染的临床疗效

摘　　要：目的探讨高频电波刀电圈切除术(LEEP)治疗宫颈上皮内瘤变(CIN)伴高危型人乳头瘤病毒(HPV)感染的临床疗效.方法选择于2018年1月~2019年12月在我院接受高频电波刀电圈切除术治疗宫颈上皮内瘤变伴高危型人乳头瘤病毒的患者共50例,手术前后所有患者都经过HR-HPV检查,通过HR-HPV指标的变化情况来判断高频电波刀电圈切除术的临床疗效.结果所有患者都顺利完成手术治疗,其中宫颈上皮内瘤变Ⅰ级患者的治愈率达到了100.00%,高于Ⅱ级患者的治愈率75.00%,差异有统计学意义(P<0.05);其中CINⅠ级患者的复发率为0,CINⅡ~Ⅲ级患者的复发率达到了26%,患者数值差异较大,明显低于治疗前的100%,差异有统计学意义(P<0.05).HR-HPV的阳性率为28.57%,转阴率为60.71%,差异有统计学意义(P<0.05).结论高频电波刀电圈切除术对于治疗宫颈上皮内瘤变伴高危型人乳头瘤病毒感染可以获得较好疗效,不仅可以消除大部分HR-HPV的感染,还可降低宫颈上皮内瘤变的复发率和宫颈癌的发生率,且操作过程简单,值得临床推广.Objective To investigate the clinical efficacy of LEEP in the treatment of cervical intraepithelial neoplasia(CIN)with high-risk human papillomavirus(HPV)infection.Methods From January 2018 to December 2019.50 patients with cervical intraepithelial neoplasia and high-risk human papillomavirus(HPV)infection were treated by LEEP in our hospital After HRHPV examination,the changes of HR-HPV index were used to judge the clinical efficacy of high-frrquency electrotome.Res u Its All patients successfully completed the surgical treatment,of which the cure rate of cervical intraepithelial neoplasia grade I patients reached(100.00)%,higher than the cure rate of gradeⅡpatients(75.00)%,the difference was statistically significant(P<0.05);the recurrence rate of cervical intraepithelial neoplasia grade I patients was 0.00,the recurrence rate of cervical intraepithelial neoplasia gradeⅡ-Ⅲpatients reached 26%,the difference was large,significanlly lower than the 100%before treatment.The difference was statistically significant(P<O.O5).The positive rate of HR-HPV was 28.57%,the negative rate was 60.71%,the difference was statistically significant(P<0.05).Conelusion Loop electrosurgical excision of cervical intraepithelial neoplasia with highrisk human papillomavirus infection can obtain good curative effect in clinical treatment,at the same time.it can eliminate most of HR-HPV infection,reduce the recurrence rate of cervical intraepithelial neoplasia and the incidenc e of cervical cancer,and the operation process is simple,which is worthy of clinical promotion.
