摘 要:目的:对比观察多波长炫彩成像(MC)和传统彩色眼底像(CFP)检查对部分眼底病变特征的检出情况。方法:回顾性分析临床确诊的眼底病患者194例208只眼。采集患眼CFP、荧光素眼底血管造影(FFA)、MC、光学相干断层扫描(OCT)图像。以FFA检查结果作为病变诊断标准,OCT作为诊断参考,对比观察MC和CFP对黄斑前膜(ERM)、黄斑裂孔(MH)、视网膜内出血(IRH)、视网膜下出血(SRH)、视网膜内积液(IRF)、视网膜下积液(SRF)、色素上皮脱离(PED)、玻璃膜疣、脉络膜新生血管(CNV)等9种常见眼底病变征象的检出情况。结果:MC对ERM(χ^(2)=12.941)、IRF(χ^(2)=29.615)、SRF(χ^(2)=16.681)、PED(χ^(2)=4.667)和CNV(χ^(2)=6.454)的检出眼数均高于CFP,差异有统计学意义(均为P<0.05);对MH(χ^(2)=1.032)、IRH(χ^(2)=1.011)、SRH(χ^(2)=1.018)和玻璃膜疣(χ^(2)=1.053)的检出眼数与CFP比较,差异无统计学意义(均为P>0.05)。GR显示浅、中层眼底病变最佳,对ERM(χ^(2)=7.333)、IRF(χ^(2)=15.763)和SRF(χ^(2)=12.102)的检出眼数均高于CFP,差异有统计学意义(均为P<0.05)。红外光反射(IR)显示视网膜外层及脉络膜层病变最佳,对PED(χ^(2)=4.667)的检出眼数高于CFP,差异有统计学意义(P<0.05)。结论:MC能够识别不同层次的眼底病变特征,提供的诊断信息较传统CFP更丰富,尤其是对ERM、IRF、SRF、PED和CNV的检出眼数高。Objective:To compare the detection of characteristics of some fundus lesions with multicolor imaging(MC) and color fundus photography(CFP).Methods:A retrospective analysis of 208 eyes of194 patients with fundus disease diagnosed by clinical examination was included in the study.All the patients were performed CFP,MC,optical coherence tomography(OCT),and fluorescein angiography(FFA) examination.Using the results of FFA as diagnostic criteria and the results of OCT as diagnostic reference,the detection of MC and CFP on epiretinal membrane(ERM),macular hole(MH),intraretinal hemorrhage(IRH),subretinal hemorrhage(SRH),intraretinal fluid(IRF),subretinal fluid(SRF),pigment epithelial detachment(PED),drusen and choroidal neovascularization(CNV) were comparatively observed.Results:The detection eyes of ERM(χ^(2)=12.941),IRF(χ^(2)=29.615),SRF(χ^(2)=16.681),PED(χ^(2)=4.667),and CNV(χ^(2)=6.454) were significantly higher in MC than CFP(P<0.05).But the detection eyes of MH(χ^(2)=1.032),IRH(χ^(2)=1.011),SRH(χ^(2)=1.018) and drusen(χ^(2)=1.053) showed no significant difference between the two methods(P>0.05).Inner and middle retinal changes were best detected on green reflectance,which detected ERM(χ^(2)-7.333),IRF(χ~=15.763) and SRF(χ^(2)=12.102) more sensitively than CFP(P<0.05).Infrared reflectance best detected outer retinal and choroidal findings,which detected PED(χ^(2)=4.667) more sensitively than CFP(P<0.05).Conclusion:MC is capable of identifying fundus lesions at various anatomical depths.The diagnostic information provided by MC is more abundant than traditional CFP,especially for ERM,IRF,SRF,PED,and CNV.
