微创整容切口联合肌瓣对腮腺肿瘤患者的效果观察

摘　　要：目的探讨联合使用胸锁乳突肌瓣的改良V形微创整容切口对患者围术期指标、美观性、术后并发症的影响。方法2016年1月至2018年12月,新疆医科大学第一附属医院颌面肿瘤外科收治良性腮腺肿瘤患者37例,男11例,女15例,年龄19~72(38.9±14.2)岁。根据手术切口不同分为改良Blair切口组26例,改良V形微创整容切口组11例。比较两组手术时间、术中出血量、住院天数、肿瘤大小。结果两组在手术时间,术中出血量,住院天数及肿瘤大小比较后,差异无统计学意义(P>0.05),改良V形微创整容切口组比改良Blair切口组显示出更好的美容效果(t=4.695,P<0.001)。结论改良V形微创整容切口术野暴露良好、切口相对隐蔽、术后美容效果良好、无明显并发症,值得临床应用。Objective To explore the effect of combined V-shaped minimally invasive cosmetic incision with sternocleidomastoid muscle flap on the perioperative indicators,aesthetics and postoperative complications of patients.Methods From January 2016 to December 2018,37 patients with benign parotid gland tumors were treated in the Department of Maxillofacial Tumor Surgery,the First Affiliated Hospital of Xinjiang Medical University,including 11 males and 15 females,aged 19-72(38.9±14.2)years.According to different surgical incisions,it was divided into modified Blair incision(26 cases)and modified V-shaped minimally invasive cosmetic incision(11 cases).The operation time,intraoperative blood loss,hospital stay and tumor size were compared between the two groups.Results After the operation time,intraoperative blood loss,hospitalization days and tumor size of the two groups,the difference was not statistically significant(P>0.05).The modified V-shaped minimally invasive cosmetic incision group showed better beauty than the modified Blair incision group Effect(t=4.695,P<0.001).Conclusions The modified V-shaped minimally invasive cosmetic incision has good surgical field exposure,relatively concealed incision,good postoperative cosmetic effect,and no obvious complications.It is worthy of clinical application.
