摘　　要：目的:输血是恶性肿瘤临床治疗的常用策略,而含有白细胞的血液制品会抑制免疫功能。胸腺素是一种免疫调节多效细胞因子,用于癌症或感染性疾病患者,提示其对癌症手术切除患者输血疗效的影响。本研究旨在探讨胸腺素在恶性肿瘤患者输血中的免疫调节作用。方法:本研究纳入80例结直肠癌患者(男47例,女33例)。将患者随机分为输注无白细胞混悬液(Thy)组和输注含白细胞混悬液(control)组。记录患者血清中CD3、CD4、CD8、CD4/CD8、NK细胞和B淋巴细胞的变化。同时记录血检、肝肾功能、体重、KPS评分等临床指标。结果:血悬液治疗前,对照组与对照组免疫功能指数(包括T细胞和NK细胞活性)、红细胞C3b受体花环率(RBC-C3bR)无明显变化(P>0.05);对照组经血悬液处理后,红细胞C3b受体花环率明显提高。同时,Thy组T3、T4、T4/T8、N K细胞活性及RBC-C3bRrate均明显提高。T3、T4、T4/T8、NK细胞活性及RBC-C3bRrate均显著高于对照组(P<0.05)。结论:本研究为临床应用胸腺素改善恶性肿瘤患者输血提供了直接证据和理论依据。Objective:Blood transfusion is a common strategy in clinical therapy of malignant tumors.However,blood products containing leukocyte can suppress immune function.Thymopentin is an immune-modulating pleiotropic cytokine that being used in patients with cancer or infectious diseases,suggesting its impact on the efficacy of blood transfusion in patients with surgically resected cancer.The aim of the present study was to assess the immune-regulating role of thymopentin in blood transfusion of patients with malignant tumors.Methods:The study included 80(47 male and 33 female)patients with colorectal cancer.Patients were randomly divided into the thymopentin receiving(Thy)group(transfusion of blood suspension without leukocyte)or the control group(transfusion of blood suspension containing leukocyte).The changes of CD3,CD4,CD8,CD4/CD8,NK cells and B lymphocytes in serum of suspension,there have no significant changes in the immune function index contains the activity of T cells and NK cells,the red blood C3b receptor garland rate(RBC-C3bR)between the control patients were recorded.In addition,the clinical index such as blood examination,liver and kidney function,body weight and KPS score were also recorded.Results:Before treatment with blood and Thy group(P>0.05);After treatment with blood suspension,the red blood C3b receptor garland rate was obviously improved in the control group.Meanwhile,the activity of T3,T4,T4/T8 and N K cells,and the RBC-C3bRrate was significantly improved in the Thy group.Additionally,the activity of T3,T4,T4/T8 and N K cells,and the RBC-C3bRrate were significantly higher than that of control group(P<0.05).Conclusion:This study provides direct evidence and rationale concerning the potential clinical use of thymopentin in improving the blood transfusion for patients with malignant tumor.
