超声乳化联合瞳孔再造术治疗急性闭角型青光眼的临床观察

摘　　要：目的:观察超声乳化联合瞳孔再造术在急性闭角型青光眼治疗中的疗效。方法:选择2017年1月~2018年1月急性闭角型青光眼60眼,随机分为两组进行前瞻性随机对照临床试验研究。超声乳化联合瞳孔再造术组(A组,30眼)和超声乳化组(B组,30眼)。末次随访时间术后1年,分别对两组术后视力、眼压、房角、瞳孔大小及畏光评分进行比较。结果:两组术后矫正视力无显著差异。两组术后1个月两组眼压无差异,术后6个月、1年A组眼压明显低于B组。A组房角开放的范围明显高于B组。A组瞳孔明显小于B组。A组畏光评分明显低于B组。结论:急性闭角型青光眼患者选择白内障摘除术+人工晶体植入术+瞳孔成形术进行治疗,疗效可靠,并发症发生率低。Objective:To observe the efficiency of phacoemul sification combined with pull reconstruction in treatmeng of acute angle-closure glaucoma.Methods:From January 2017 to January 2018,60 eyes of acute angle-closure glaucoma were selected and randomly divided into two groups for prospective randomized controlled clinical trial research.Phacoemulsification combined with pupil reconstruction group(group A,30 eyes)and phacoemulsification group(group B,30 eyes).The last follow-up time was 1 year after surgery,and the postoperative visual acuity,intraocular pressure,angle of the chamber,pupil size and photophobia scores were compared between the two groups.Results:There was no significant difference in postoperative corrected visual acuity between the two groups.There was no difference in intraocular pressure between the two groups at 1 month after operation.The intraocular pressure in group A was significantly lower than that in group B at 6 months and 1 year after operation.The range of angle opening in group A was significantly higher than that in group B.The pupils of group A were significantly smaller than those of group B.The photophobia score of group A was significantly lower than that of group B.Conclusion:The efficiency of phacoemul sification combined with pull reconstruction in treatmeng of acute angle-closure glaucoma is reliable.In addition,there are fewer postoperative complications,the methods of operation is worth clinically.
