白内障超声乳化吸除术联合小梁切除术治疗闭角型青光眼并白内障的临床疗效分析

摘　　要：目的分析白内障超声乳化吸除术与人工晶状体植入术分别联合小梁切除术治疗闭角型青光眼并白内障的临床疗效。方法80例闭角型青光眼并白内障患者,采用随机抽签法分为对照组与观察组,各40例。对照组以人工晶状体植入术联合小梁切除术治疗,观察组以白内障超声乳化吸除术联合小梁切除术治疗。对比两组患者术前及术后6个月的视力、眼压、散光度、前房角宽度及前房深度变化情况。结果术后6个月,观察组患者眼压、视力以及散光度分别为(13.0±2.1)mm Hg(1 mm Hg=0.133 kPa)、(0.8±0.2)、(0.7±0.3)D均优于对照组的(16.4±2.5)mm Hg、(0.6±0.1)、(0.8±0.1)D,差异均具有统计学意义(P<0.05)。术后6个月,两组患者前房角宽度及前房深度均优于本组术前,差异具有统计学意义(P<0.05);两组患者术前及术后6个月前房角宽度及前房深度对比,差异无统计学意义(P>0.05)。结论采用白内障超声乳化吸除术联合小梁切除术治疗闭角型青光眼并白内障,疾病治疗效果好,可有效改善患者视力及临床症状,降低眼压,临床应用价值高。Objective To analyze the clinical efficacy of phacoemulsification and intraocular lens implantation combined with trabeculectomy in the treatment of angle closure glaucoma with cataract.Methods A total of 80 cases of angle closure glaucoma with cataract were divided into control group and observation group according to random lottery method,with 40 cases in each group.The control group was treated with intraocular lens implantation combined with trabeculectomy,and the observation group was treated with phacoemulsification combined with trabeculectomy.The visual acuity,intraocular pressure,astigmatism,width of the anterior chamber and depth of the anterior chamber before and 6 months after the operation of the two groups were compared.Results 6 months after the operation,the visual acuity,intraocular pressure,astigmatism of the observation group were(13.0±2.1)mm Hg(1 mm Hg=0.133 kPa),(0.8±0.2)and(0.7±0.3)D,which were all better than(16.4±2.5)mm Hg,(0.6±0.1)and(0.8±0.1)D of the control group,and the difference was statistically significant(P<0.05).6 months after the operation,the width and depth of the anterior chamber of the two groups were all better than those before operation,and the difference was statistically significant(P<0.05).Before and 6 months after the operation,there was no statistically significant difference in width and depth of the anterior chamber in the two groups(P>0.05).Conclusion Phacoemulsification combined with trabeculectomy shows good effect in the treatment of angle closure glaucoma with cataract,which can improve the visual acuity and clinical symptoms and reduce intraocular pressure,and has high application value.
