摘　　要：目的分析血清甲胎蛋白异质体(AFP-L3)联合α-L岩藻糖苷酶(AFU)检测小肝癌的诊断价值。方法选取2017年2月—2019年2月赣州市肿瘤医院收治的100例疑似小肝癌患者作为研究对象,均进行6个月随访,根据随访结束时病症的不同分为A组(小肝癌组,40例)、B组(肝脏良性病变组,60例)。2组患者均于随访结束时检测血清AFP-L3和AFU水平,绘制受试者工作曲线(ROC),通过曲线下面积(AUC)评价血清AFP-L3和AFU联合检测诊断小肝癌的价值。结果随访结束时,A组患者中AFP-L3及AFU阳性率均高于B组,差异具有统计学意义(P<0.05);绘制ROC曲线,结果显示AFP-L3、AFU及联合检测诊断小肝癌的AUC为0.913,0.900,0.921,其中联合检测诊断价值更优。结论小肝癌患者有着较高的血清AFP-L3和AFU阳性率,且AFP-L3和AFU联合检测较单一指标检测的诊断价值更高。Objective To investigate the diagnostic value of combined detection of serum alpha-fetoprotein(AFP)heterogeneous body andα-L fucosidase(AFU)for small hepatocellular carcinoma.Methods A total of 100 patients with suspected small liver cancer treated in our hospital from February 2017 to February 2019 were selected as the research subjects,and all were followed up for 6 months.According to the different symptoms at the end of the follow-up,they were divided into group A(small liver cancer group,40 cases)and group B(hepatic benign lesion group,60 cases).Both groups of patients were tested for serum AFP-L3 and AFU levels at the end of follow-up;the receiver operating curve(ROC)was drawn,and the area under the curve(AUC)was used to evaluate the value of combined detection of serum AFP-L3 and AFU in the diagnosis of small liver cancer.Results At the end of the follow-up,the positive rates of AFP-L3 and AFU in group A were higher than those in group B,and the difference was statistically significant(P<0.05);the ROC curve was drawn,and the results showed that the AUC of AFP-L3,AFU and combined detection for small liver cancer were 0.913,0.900,0.921,among which the combined detection had better diagnostic value.Conclusion Patients with small liver cancer have a higher positive rate of serum AFP-L3 and AFU,and the combined detection of AFP-L3 and AFU is more effective in diagnosing small liver cancer than a single index detection,and the diagnostic value is higher.
