磁共振FLASH序列和采用CAIPIRINHA加速的3D-T_(1)-SPACE序列对颅脑肿瘤成像质量的比较研究

摘  要:目的探讨磁共振二维快速小角度激发(2D-FLASH)序列(以下简称FLASH序列)和受控混叠导致更高加速度的并行成像加速的三维T1加权可变翻转角度下最佳对比度的应用与采样完善(3D-CAIPIRINHA-T1-SPACE)序列(以下简称SPACE序列)对颅脑肿瘤图像质量的差异。方法27例患者在我院行头颅MRI增强检查,接受造影剂注射后随机排列矢状位、冠状位和横断位FLASH序列和矢状位SPACE序列。两名高年资影像诊断医师对两种序列图像进行独立分析,采用4分评分标准评价不同磁共振序列显示脑肿瘤的图像质量。测量两组图像的信号强度,计算病灶感兴趣区、灰质和白质的信噪比(SNR)、病灶相对于脑白质和脑灰质的对比度噪声比(CNR)、脑组织灰质相对于白质的CNR。结果27例患者共31次检查,共测量病灶41个。SPACE序列显示颅脑肿瘤图像质量的主观评分高于FLASH序列(P<0.05)。SPACE序列图像的病灶、灰质和白质的SNR明显高于FLASH序列图像(P<0.05)。SPACE序列图像的病灶对白质和病灶对灰质的CNR明显高于FLASH序列图像(P<0.05)。FLASH序列图像灰质对白质的CNR高于SPACE序列图像(P<0.05)。结论增强后SPACE序列图像优于FLASH序列图像。Objective To explore the differences of image quality between 2D fast low angle shot(2D-FLASH)sequence(hereinafter referred to as FLASH)and 3D T1 sampling performance with application optimized contrast using different flap angle accelerated by controlled aliasing in parallel imaging results in higher acceleration(3D-CAIPIRINHA-T1-SPACE)sequence(hereinafter referred to as SPACE)in brain tumors.Methods Twenty-seven patients underwent contrast-enhanced MRI of the head in our hospital.After contrast agent injection,the sagittal,coronal and transverse FLASH sequences and sagittal SPACE sequences were randomly arranged.Two senior imaging diagnosticians independently analyzed the images of the two sequences and evaluated the image quality of different MRI sequences by using the 4-point scoring criteria.The signal strength of two groups of images were measured,the signal-to-noise ratio(SNR)of lesions ROI,gray matter and white matter,the contrast to noise ratio(CNR)of lesions to white matter and gray matter,and CNR of gray matter to white matter were calculated.Results A total of 31 times examination were performed in 27 patients and 41 lesions were measured.The subjective score of brain tumor image quality of SPACE sequence was higher than that of FLASH sequence(P<0.05).The SNR of lesions,gray matter and white matter in SPACE sequence images were significantly higher than those in FLASH sequence images(P<0.05).The CNR of lesion to grey matter and lesion to white matter in SPACE sequence images were significantly higher than those in FLASH sequence images(P<0.05).The CNR of gray matter to white matter in FLASH sequence images was higher than that in SPACE sequence images(P<0.05).Conclusion The images of contrast-enhanced SPACE sequence is better than that of FLASH sequence.
