梨状窝尖在判断梨状窝癌侵犯下咽部环后段的意义

摘　　要：目的通过观察梨状窝癌患者侵犯下咽部环后段时梨状窝尖的受累情况,探讨梨状窝尖与梨状窝癌侵犯下咽部环后段的相关性。方法回顾性分析环后段受到侵犯的梨状窝癌患者26例,观察梨状窝尖的累及情况。并对不同类型梨状窝癌侵犯梨状窝尖的几率进行Fisher精确检验。结果26例中,19例侵犯了梨状窝尖,外侧型癌侵犯梨状窝尖的几率较大,(P<0.05)为差异有统计学意义。结论大部分侵犯环后段的梨状窝癌均侵犯了梨状窝尖,梨状窝尖是病变向环后段侵犯的重要标志。Objective To investigate the involvement of pyriform sinus apex when pyriform sinus carcinoma invading the postcricoid area of hypopharynx in order to explore the correlation between the piriform sinus apex and the postcricoid area of hypopharynx invaded by pyriform sinus carcinoma.Methods A retrospective analysis was conducted on 26 patients with piriform sinus carcinoma who were invaded in postcricoid area of hypopharynx to observe the involvement of the piriform sinus apex.Fisher's precise test was performed on the probability of different types of pyriform sinus cancer invading the piriform sinus apex.Results In 26 cases of piriform sinus carcinoma that invaded postcricoid area of hypopharynx,19 cases invaded the piriform sinus apex.In the lateral type preferred to invade the pyriform fossa apex,there was statistical difference(P<0.05).Conclusion When postcricoid area of hypopharynx was invaded by pyriform sinus carcinoma,most of these cases have an invasion in the piriform sinus apex.The piriform sinus apex is an important marker for the invasion of the lesion to the postcricoid area of hypopharynx.
