ADC值和相对ADC鉴别乳腺良恶性病变的价值

摘　　要：目的探讨ADC值和相对ADC(relative ADC,rADC)鉴别乳腺良恶性病变的价值。方法回顾性分析2011年1月~2018年12月济宁医学院附属医院通过病理或活检证实的乳腺病变,其中良性病变组31例,恶性病变组41例,测量病变区ADC值及其周围正常腺体ADC值,并计算rADC(病变ADC值/周围正常腺体ADC值),将良性病变组ADC值和恶性病变组ADC值;良性病变组rADC和恶性病变组rADC分别做独立样本t检验,比较组间差异,并绘制ROC曲线。结果良性病变组ADC(1.33±0.24)×10^(-3) mm^(2)/s,恶性病变组ADC(0.94±0.25)×10^(-3) mm^(2)/s(t=-6.755,P<0.001);良性病变组rADC 0.71±0.12,恶性病变组rADC 0.50±0.12,(t=-7.389,P<0.001)。ADC值最佳诊断分界点为1.264×10^(-3) mm^(2)/s,ROC曲线下面积为0.856,灵敏度为80.5%,特异度为80.7%,rADC最佳诊断分界点为0.624,ROC曲线下面积为0.893,灵敏度为85.4%,特异度为90.3%。结论ADC值、rADC可以鉴别乳腺良恶性病变,rADC的诊断准确性更高。Objective To evaluate the role of the apparent diffusion coefficient(ADC)values and relative apparent diffusion coefficient in distinguishing benign from malignant breast lesions.Methods 72 breast lesions,consisting of 41 malignant lesions and 31 benign lesions,which were proved by surgery or biopsy from January 2011 to December,2018,were analysed retrospectively.The ADC values of these lesions were measured,also in normal tissue around,the rADC(lesion ADC/normal tissue around ADC)was calculated.The t-test was used for testing the difference of ADC values between benign and malignant groups,as well as rADC.Threshold of ADC and rADC for differential diagnosis was acquired by ROC analysis.Results The ADC values of benign and malignant lesions were(1.33±0.24)×10^(-3) mm^(2)/s and(0.94±0.25)×10^(-3) mm^(2)/s(t=-6.755,P<0.001).The rADC values were(0.71±0.12)and(0.50±0.12)(t=-7.389,P<0.001).The thresholds of ADC,rADC were 1.264×10^(-3) mm^(2)/s,0.624.The AUC,sensitivity and specificity of ADC and rADC were 0.856,80.5%,80.7%;0.893,85.4%,90.3%.Conclusion ADC values and rADC are of important significance in differentiating benign and malignant breast lesions;especially,rADC values have higher diagnostic accuracy.
