摘　　要：目的探讨结肠癌CT纹理分析与术后TNM分期的相关性。方法回顾性分析99例结肠癌患者的术前CT图像。选取横断位图像病灶最大层面手动勾画ROI。通过Mann-Whitney U检验评估不同TNM分期的纹理参数差异性。采用ROC曲线分析评估CT纹理参数在区分TNM不同分期的诊断效能。结果静脉期min在T1-2和T3-4分期组中差异有统计学意义(P=0.042),AUC值(曲线下面积)为0.706(P=0.0105)。静脉期的max frequency在区分N-和N+分期以及I~II和III~IV分期差异有统计学意义(P<0.05),AUC值分别为0.626,0.616(P<0.05)。不同M分期中,静脉期的mean、min、max、percentile(5,10,25,75,90)、ssD low差异有统计学意义(均P<0.05),AUC值为0.701~0.760,(均P<0.05)。结论静脉期CT纹理分析参数可为评估结肠癌TNM分期提供参考。Objective To explore the correlation between computed tomography(CT)texture analysis and postoperative TNM stages of colon cancer.Methods Preoperative CT images of 99 patients with colon carcinomas were reviewed retrospectively.Regions of interest(ROIs)were manually drawn along the margin of the tumor on the maximum slice of cross-sectional images.The Mann-Whitney U test was used to evaluate the differences in texture parameters of different TNM stages.The diagnostic performance of CT texture parameters in differentiating various TNM stages was evaluated using receiver operating characteristic(ROC)curve analysis.Results The parameter min of venous phase was significantly different between the T1-2 and T3-4 stages(P=0.042),and the AUC value(area under the curve)was 0.706(P=0.0105).The max frequency of the venous phase was significantly different not only between the N-and N+stages but also the stages I~II and III~IV(P<0.05),and the AUC values were 0.626 and 0.616(P<0.05),respectively.In different M stages,mean,min,max,percentile(5,10,25,75,90)and ssD low of the venous phase were significantly different(all P<0.05),the AUC values were 0.701~0.760(P<0.05).Conclusion CT texture analysis parameters in venous phase may provide a helpful reference for the evaluation of TNM stages of colon cancer.
