抗Hp治疗对AFLD伴Hp感染临床疗效的影响

摘　　要：目的探讨抗幽门螺杆菌(Hp)治疗对酒精性脂肪性肝病(AFLD)伴Hp感染患者疗效。方法选取本院住院和门诊患者明确诊断为AFLD伴Hp感染患者共87例,随机分为初始治疗组(46例)和对照组(41例)。在保肝、对症治疗基础上,初始治疗组给予抗Hp 2周。抗Hp完成1个月后,初始治疗组检查碳呼气试验,剔除Hp仍阳性5例患者,构成Hp根除者治疗组、对照组均为41例。抗Hp结束2个月后,记录两组治疗前后消化道症状改善情况,检查肝功能、血脂、肝纤维化四项,抗Hp结束6个月后复查腹部肝脏彩超。行相关统计分析。结果抗Hp治疗结束后,两组在症状改善方面差异有统计学意义(P均<0.05),且在肝功能、血脂、肝纤维化好转方面差异也具有统计学意义(P均<0.05)。两组治疗总有效率比较差异有统计学意<(P<0.05)o结论AFLD伴Hp感染者抗Hp治疗,有利于改善其临床症状,恢复肝功能,稳定血脂水平状况,进一步修复肝脏脂肪病变,达到减少并发症及改善可能预后目的。Objective To explore the impact of anti-Helicobacter Pylori(Hp)regimen on patients who suffered Alcoholic Fatty Liver Disease(AFLD)and HP infection.Methods Eighty-seven patients with confirmed diagnosis of AFLD and Hp test positive were invited.They were from both the inpatient and out-patient departments in a hospiul.The patients were randomly grouped,with 46 patients in the therapy group and 41 patients in the control group.The two groups were administered with the routine treatments aiming to enhance liver function and to relieve clinical symptoms.The therapy group were further given anti-Hp treatment for 14 days.One month post anti-HP treatment,C14 urea breath test was performed.Five of the 46 patients tested positive and they were excluded from the therapy group.Two months post anti-Hp treatment,gastric symptoms,liver functions,blood lipid and liver fibrosis indexes of the two groups were collected and compared.Six months post Hp treatment,a color liver ultrasound was performed with all the patients.Results Patients in the therapy group showed statistically improved gastrointestinal symptoms(P<0.05).Liver function,blood lipid and four indexes of liver fibrosis in the therapy group patients statistically improved compared with the control group(P<0.05).The overall effectiveness of the treatment in the therapy group was statistically better(P<0.05).Conclusion Anti-Hp regimen is effective in improving clinical symptoms of AFLD patients with Hp infection.The treatment can improve the patients*liver functions,reduce their blood lipid levels,decrease their complications and improve their prognosis.
