溃疡性结肠炎患者病情不确定感与负性情绪的关系

摘　　要：目的探讨溃疡性结肠炎病情不确定感与焦虑、抑郁等负性情绪的关系。方法研究对象为2018年2月~2020年2月我院80例溃疡性结肠炎患者,采用Mishel疾病不确定感问卷中文版(MUIS)评价患者病情不确定感状况,并比较不同水平病情不确定感的患者其汉密尔顿抑郁量表(HAMD)以及汉密尔顿焦虑量表(HAMA)的差异,Pearson相关分析探讨MUIS评分与HAMD、HAMA评分的相关性。结果 80例溃疡性结肠炎患者MUIS总分为(58.93±6.87)分,低水平23例,中等水平38例,高水平19例;病情不确定感高水平组HAMD评分、HAMA评分高于中、低水平组,中等水平组HAMD评分、HAMA评分高于低水平组,差异具有统计学意义(P<0.05);溃疡性结肠炎患者MUIS量表复杂性、不确定性以及总分均与HAMD评分、HAMA评分呈正相关关系(P<0.05)。结论溃疡性结肠炎患者普遍存在病情不确定感,以中等水平为主,且与负性情绪具有明显相关性,临床应采取相应措施降低患者病情不确定感,以期改善负性情绪。Objective To explore the relationship between the illness state uncertainty of ulcerative colitis and negative emotions such as anxiety and depression. Methods Eighty patients with ulcerative colitis in our hospital from February 2018 to February 2020 were selected as the study subjects. The Chinese version of the Mishel disease uncertainty questionnaire(MUIS)was used to evaluate the status of patient’s illness state uncertainty. The differences between Hamilton depression scale(HAMD)and Hamilton Anxiety Scale(HAMA)in patients with different levels of illness state uncertainty were compared. Pearson correlation analysis were used to explore the correlation between MUIS score and HAMD and HAMA scores. Results The total MUIS score of 80 patients with ulcerative colitis was(58.93±6.87),23 were low-level,38 were medium-level,and 19 were high-level. The HAMD and HAMA scores in the high-level group of illness state uncertainty were higher than those in the middle and low-level group(P<0.05). The HAMD and the HAMA scores in the middle-level group was higher than those in the low-level group(P<0.05). The complexity,uncertainty and total score of the MUIS scale were positively correlated with HAMD score and HAMA score in patients with ulcerative colitis(P<0.05). Conclusion Patients with ulcerative colitis generally have a sense of uncertainty in their illness,which is mainly at a moderate level,and has a significant correlation with negative emotions. Corresponding measures should be taken clinically to reduce the uncertainty of patients’ condition in order to improve negative emotions.
