摘　　要：目的分析早孕期胎儿全前脑面部畸形产前超声诊断的临床价值。方法连续收集2015年1月至2019年12月在湖南省妇幼保健院行早孕期产前超声检查的30例全前脑畸形胎儿,以引产后的病理诊断结果为标准,记录各种类型全前脑面部畸形的产前超声诊断符合率,总结全前脑面部畸形的产前超声图像特点。结果 30例全前脑畸形中29例(96.67%)伴有明显的面部畸形,并有不同组合方式;产前超声对鼻骨发育不良的诊断符合率为100.00%(29/29),对无眼的诊断符合率为100.00%(2/2),对眼距窄的诊断符合率为95.65%(22/23),对中央型唇腭裂的诊断符合率为75.00%(9/12)。结论产前超声早孕期胎儿鼻骨发育不良、无眼、眼距窄及中央型唇腭裂的诊断率较高,分析超声图像特点对提高早孕期胎儿全前脑畸形产前诊断率具有临床价值。Objective To analyze the clinical value of prenatal ultrasonography in the diagnosis of fetal holoprosencephaly during early pregnancy.Methods Thirty fetuses of fetal holoprosencephaly who underwent prenatal ultrasonography during early pregnancy were continuously collected in Hunan Maternal and Child Health Hospital from January 2015 to December 2019.The pathological diagnosis after induced labor was taken as the standard,the coincidence rate of prenatal ultrasonography in the diagnosis of various types of holoprosencephaly was recorded,and the characteristics of prenatal ultrasonography images of holoprosencephaly were summarized.Results Among the 30 cases of fetal holoprosencephaly,29(96.67%)were accompanied by obvious facial malformations,which had different combinations.The diagnostic coincidence rates of prenatal ultrasonography for nasal bone dysplasia,anopia,ocular hypotelorism,central cleft lip and palate were 100.00%(29/29),100.00%(2/2),95.65%(22/23),and 75.00%(9/12),respectively.Conclusion The diagnostic rates of prenatal ultrasonography were higher on fetal nasal bone dysplasia,anopia,ocular hypotelorism,central cleft lip and palate during early pregnancy.The analysis of the characteristics of prenatal ultrasonography images had clinical value in improving the diagnostic coincidence rates of holoprosencephaly during early pregnancy.
