摘  要:目的探索帕金森病(Parkinson disease,PD)患者肠道菌群组成的变化及其与临床特征的关系。方法根据一定入选和排除标准选择30例临床诊断的PD患者,以22例健康人作为对照组,基于粪便菌群的16S rDNA基因V3-V4区测序,分析比较PD患者与健康人群肠道菌群组成和丰度的差异,并分析不同临床表现与PD患者肠道菌群之间的相互联系。结果①与健康对照组相比较,PD患者肠道菌群多样性增高(P<0.05)。②在菌群组成方面,门水平,PD患者厚壁菌门、放线菌门、变形菌门丰度明显高于对照组(P<0.05),而拟杆菌门丰度明显减低,差异均有统计学意义(P<0.05)。在科水平,PD组双歧杆菌科、肠杆菌科、瘤胃菌科、链球菌科、草酸杆菌科、紫单胞菌科明显丰度高于对照组(P<0.05),韦荣球菌科丰度明显减低(P<0.05)。③PD患者肠道菌群组成及丰度在早发及晚发型、震颤与非震颤型、伴发便秘与不伴发便秘组菌群多样性均未见差异(P>0.05)。④相关性分析表明在门水平,厚壁菌门丰度分别与UPDRS Ⅲ评分、NMSS评分、Wexner评分、PDQ-39评分均呈正相关(r=0.38,0.35,0.44,0.35;P<0.05);放线菌门丰度与UPDRS Ⅲ、NMSS、Wexner及PDQ-39的评分均呈正相关(r=0.40,0.47,0.28,0.48;P<0.05);拟杆菌门丰度与UPDRS Ⅲ、NMSS、Wexner、PDQ-39的评分均呈负相关(r=-0.45,-0.45,-0.51,-0.43;P<0.05)。在科水平,双歧杆菌科、肠杆菌科、紫单胞菌科的丰度分别与UPDRS Ⅲ、NMSS、PDQ-39均呈正相关(r=0.35,0.42,0.44;r=0.30,0.29,0.30;r=0.47,0.42,0.50;均P<0.05)。结论与健康人相比较,PD患者肠道菌群多样性增加,双歧杆菌科、肠杆菌科和紫单胞菌科与PD的运动症状和非运动症状的严重程度有关。Objective To analyze the changes of microbial composition in Parkinson’s disease(PD)patients,and to study the relationships between clinical phenotypes and intestinal microbiota.Methods According to inclusion and exclusion criteria,30 patients with PD and 22 healthy controls were selected.Based on the sequencing of V3-V4 region of 16S rDNA gene in fecal flora,the diffe-rences of intestinal flora composition and abundances between PD patients and healthy controls were analyzed.The relationship between different clinical types and intestinal microbiota in patients with PD were also analyzed.Results①The diversity of intestinal microbiota in PD patients was significantly higher than that in healthy controls(P<0.05).②At the phylum level,the abundances of Firmicutes,Actinobacteria,Proteobacteria in PD patients were significantly higher than those in healthy controls,while the abundance of Bacteroidetes was significantly lower(all P<0.05).At the taxonomic level of family level,the abundances of Bifidobacteriaceae,Enterobacteriaceae,Ruminococcaceae,Streptococcaceae,Oxalobacteraceae,Porphyromonadaceae were significantly higher than those in healthy controls,while the abundance of Veillonellaceae was significantly lower(P<0.05).③There was no significant difference in the composition and abundances of intestinal flora between early and late PD patients,between tremor and non-tremor PD patients,between PD patients with constipation and without constipation group(P>0.05).④At the phylum level,there was a positive correlation between the scores of UPDRS Ⅲ,NMSS,Wexner and PDQ-39 and abundances of Firmicutes and Actinobacteria(r=0.38,0.35,0.44,0.35;r=0.40,0.47,0.28,0.48;P<0.05),and a negative correlation between the scores of UPDRS Ⅲ,NMSS,Wexner,PDQ-39 and abundances of Bacteroidetes(r=-0.45,-0.45,-0.51,-0.43;P<0.05).At the family level,there was a positive correlation between the scores of UPDRS Ⅲ,NMSS,and PDQ-39 and abundances of Bifidobacteriaceae,Enterobacteriaceae,Porphyromonadaceae(r=0.35,0.42,0.44;r=0.30,0.
