细胞角蛋白19片段抗原、糖抗原19-9与鳞状细胞癌抗原联合检测在食管癌诊断中的价值研究

摘　　要：目的:探讨细胞角蛋白19片段抗原(CYFRA21-1)、糖抗原19-9(CA_(19-9))与鳞状细胞癌抗原(SCC-Ag)联合检测在食管癌诊断中的价值。方法:选取食管癌患者140例为观察组,同时选取同期健康体检者90例为对照组。检测两组外周血CYFRA21-1、CA_(19-9)及SCC-Ag并进行比较,分析联合检测对食管癌的诊断价值。结果:观察组手术前后CYFRA21-1、CA_(19-9)与SCC-Ag水平均高于对照组,且手术后各指标水平均低于手术前(均P<0.05)。观察组中发生复发转移者的CYFRA21-1、CA_(19-9)与SCC-Ag水平均高于手术前(均P<0.05)。随着临床分期的递增,观察组患者CYFRA21-1、CA_(19-9)与SCC-Ag水平均呈升高趋势(均P<0.05)。CYFRA21-1、CA_(19-9)与SCC-Ag三种指标联合诊断食管癌的阳性率明显高于各单项检测(均P<0.05)。结论:血清CYFRA21-1、CA_(19-9)与SCC-Ag联合检测相对于单项检测在食管癌诊断中的价值更高,可提高疾病检出率。Objective:To explore the value of combined detection of CYFRA21-1,CA 19-9 and SCC-Ag in the diagnosis of esophageal cancer.Methods:A total of 140 patients with esophageal cancer were selected as the observation group,and 90 patients who underwent a physical examination during the same period were selected as the control group.The peripheral blood CYFRA21-1,CA 19-9 and SCC-Ag were detected and compared between the two groups,and the diagnostic value of combined detection for esophageal cancer was analyzed.Results:The levels of CYFRA21-1,CA 19-9 and SCC-Ag in the observation group before and after operation were higher than those in the control group,and the levels of each index after the operation were lower than those before the operation(all P<0.05).The levels of CYFRA21-1,CA 19-9 and SCC-Ag in the observation group who had recurrence and metastasis were higher than those before surgery(all P<0.05).With the increasing clinical stage,the levels of CYFRA21-1,CA 19-9 and SCC-Ag in the observation group showed an increasing trend(all P<0.05).The positive rate of CYFRA21-1,CA 19-9 and SCC-Ag in the combined diagnosis of esophageal cancer was significantly higher than that of single test(all P<0.05).Conclusion:The combined detection of serum CYFRA21-1,CA 19-9 and SCC-Ag is more valuable than single detection in the diagnosis of esophageal cancer,which can increase the disease detection rate.
