盐酸曲马多缓释片辅助治疗肝癌疼痛的疗效及作用机制

摘　　要：目的探讨盐酸曲马多缓释片辅助治疗肝癌疼痛的疗效及作用机制。方法选取2017年2月至2020年2月于东莞市松山湖中心医院治疗的80例肝癌患者作为研究对象,采用随机数字表法分为观察组与对照组,各40例。对照组给予羟考酮缓释片、吗啡缓释片、吗啡片、芬太尼透皮贴剂等一种或两种镇痛药物,观察组给予盐酸曲马多缓释片或盐酸曲马多注射剂,比较两组镇痛疗效、起效与持续时间、生命质量、抑郁情况及不良反应。结果观察组镇痛有效率明显低于对照组(P<0.05),起效时间长于对照组(P<0.05),持续时间短于对照组(P>0.05);治疗前两组患者生活质量评分(QOL)及抑郁自制量表(SDS)评分比较差异无统计学意义(P>0.05),治疗后观察组QOL评分明显高于对照组(P<0.05),SDS评分明显低于对照组(P<0.05);观察组不良反应发生率明显低于对照组(P<0.05)。结论盐酸曲马多缓释片是通过抑制中枢神经系统下行传导疼痛及平滑肌痉挛来辅助治疗肝癌疼痛的,其虽镇痛效果较弱,在药效发挥方面不占优势,但不良反应较少,可提升生命质量,改善抑郁情绪。Objective To explore the efficacy and mechanism of tramadol hydrochloride sustained-release tablets in the adjuvant treatment of liver cancer pain.Methods A total of 80 patients with liver cancer treated in Songshanhu Central Hospital of Dongguan City from February 2017 to February 2020 were selected as the research objects and were divided into a control group and a study group,with 40 cases in each group.The control group was given oxycodone sustained release tablets,morphine sustained release tablets,morphine tablets,fentanyl transdermal patch one or other analgesics.The observation group was given tramadol hydrochloride sustained-release tablets or tramadol hydrochloride injection.The analgesic efficacy,onset and duration,quality of life,depression and adverse reactions were compared between the 2 groups.Results The effective rate of analgesia in the observation group was significantly lower than that in the control group(P<0.05),the onset time was longer than that in the control group(P<0.05),and the duration was shorter than that in the control group(P>0.05);there was no significant difference between QOL and SDS scores(P>0.05).After treatment,the QOL score of the observation group was significantly higher than that of the control group(P<0.05),and the SDS score was significantly lower than that of the control group(P<0.05);the incidence of adverse reactions in the observation group was significantly lower than that in the control group(P<0.05).Conclusion Tramadol hydrochloride sustained-release tablets are used to assist the treatment of liver cancer pain by inhibiting the downward conduction pain of the central nervous system and smooth muscle spasm.Although its analgesic effect is weak,it does not have an advantage in the efficacy of the drug.It has fewer adverse reactions and can improve the quality of life and improve depression.
