摘  要:目的评价虹膜拉钩辅助人工晶状体(IOL)单襻固定在外伤性白内障伴晶状体不全脱位手术中应用的效果。方法回顾性分析安庆市第二人民医院2013年2月至2019年11月外伤性白内障伴晶状体不全脱位14例(14眼)的临床资料。所有患者采用虹膜拉钩固定囊袋后,行超声乳化IOL囊袋内植入及单襻巩膜缝线固定术。术后随访6个月。观察术后视力,术中及术后并发症。结果术中玻璃体脱出者5眼(35.71%,5/14)。术后最佳矫正视力为0.2~0.8,其中≥0.5者10眼(71.43%,10/14)。有2眼发生后发性白内障。IOL均无明显偏位。结论应用虹膜拉钩固定囊袋,提高外伤性白内障伴晶状体不全脱位手术安全性。IOL单襻巩膜缝线固定可保持术后IOL的长期稳定。Objective To evaluate the efficacy of application of iris retractor assisted single-haptic scleral fixation of intraocular lens in the treatment of traumatic cataract with subluxation of lens.Methods Fourteen eyes of 14 cases with traumatic cataract with subluxation of lens from Feb.2013 to Nov.2019 in the Second People's Hospital of Anqing were analyzed retrospectively.All cases were fixed the capsular with iris hook and then received phacoemulsification,IOL capsular implantation and single-haptic scleral fixation.The follow-up time was 6 months.Postoperative visual acuity,intraoperative and postoperative complications were obsrved.Results Vitreous prolapsed during operation occurred in 5 eyes(35.71%,5/14).The postoperative best corrected visual acuity was ranged from 0.2 to 0.8.The visual acuity was≥0.5 in 10 eyes(71.43%,10/14).There were 2 eyes with after cataract at 3 months after operation.The location of IOL was in the center and no dislocation or decentration was found.Conclusion Application of iris retractor could maintain the stability of the capsular bag and improve the safety of the operation.Single-haptic suture fixation of intraocular lens is safe and stable for the lens subluxation.
