地塞米松及山莨菪碱球后注射治疗复视效果观察

摘　　要：目的观察地塞米松及山莨菪碱球后注射对复视的治疗效果。方法采用回顾性研究,选取2016年6月至2019年5月于烟台业达医院就诊的复视168例,按患者自行选择治疗方式分为两组。对照组,72例,采用静脉滴注血栓通及三磷酸胞苷二钠治疗;试验组,96例,除以上治疗外,再行球后注射地塞米松及山莨菪碱。治疗后随访6个月,比较两组疗效。结果治疗后1周试验组有效率为89.58%(86/96),高于对照组的70.83%(51/72)(x^(2)=9.612,P=0.002)。试验组眼球运动平均改善(1.48±0.32)mm,优于对照组的(1.13±0.22)mm(t=3.020,P=0.003)。两组患者斜视度改善情况,差异有统计学意义(P<0.05),最大分离距离变化情况差异有统计学意义(t=2.550,P=0.012),结论地塞米松及山莨菪碱球后注射对复视疗效优于单独全身用药。Objective To observe the efficacy of retrobulbar injection of dexamethasone and racemic anisodamine for diplopia.Methods This was a retrospective study.A total of 168 patients with diplopia who were treated from Jun.2016 to May 2019 in Yeda Hospital of Yantai were collected.The patients were divided into trial group and control group based on different treatment options.The 72 cases in the control group were treated with intravenous infusion of Xueshuantong and cytidine disodium triphosphate.Besides this treatment,the 96 cases in the trial group were treated with retrobulbar injection of dexamethasone and anisodamine.The efficacy were compared between the two groups,and the follow-up time was 6 months after the treatment.Results The effective rate was 89.58%(86/96)in the trial group,which was higher than 70.83%(51/72)in the control group at 1 week after treatment(x^(2)=9.612,P=0.002).The average improvement of eye movement in the trial group was(1.48±0.32)mm,which was higher than(1.13±0.22)mm in the control group(t=3.020,P=0.003).After treatment,the difference in the improvement of the strabismus degree between the two groups were statistically significant(P<0.05).There was a statistically significant difference in the maximum separation distance changes between the two groups before and after treatment(t=2.550,P=0.012).Conclusion Retrobulbar injection of dexamethasone and racemic anisodamine for the treatment of diplopia is superior to systemic medication alone.
