无染色剂辅助内界膜剥除术治疗特发性黄斑孔的效果观察

摘　　要：目的评价无染色剂辅助的内界膜剥除术治疗特发性黄斑孔的临床效果。方法回顾性分析郑州市第二人民医院2018年7月至2019年7月特发性黄斑孔84例(84眼)的临床资料。所有患者分为2组,无染色组46眼,ICG染色组38眼。所有患者均采用25 G微创玻璃体切除术,术中剥除内界膜,均以非膨胀气体填充。观察术后视力及两组黄斑孔的闭合率。术后随访3-6个月。结果无染色组黄斑孔闭合率为97.83%(45/46),与染色组的100.00%(38/38)相近(x^(2)=0.836,P=0.361)。无染色组术后3个月及6个月视力(BCVA,log MAR)优于术前(F=7.408,P=0.001)。两组术后视力差异无统计学意义(P>0.05)。结论特发性黄斑孔采用无染色剂辅助内界膜剥除术,可避免染色剂毒副作用,促进术后视功能恢复,是一种安全有效的治疗方法。Objective To evaluate the clinical efficacy of internal limiting membrane peeling without assisted staining for idiopathic macular hole(1MH).Methods The clinical data of 84 eyes of 84 cases with IMH from Jul.2018 to Jul.2019 in the Second People's Hospital of Zhengzhou were retrospectively analyzed.Those patients were divided into two groups,including no staining group with 46 eyes of 46 patients and ICG staining group with 38 eyes of 38 patients.All cases underwent 25G minimally invasive vitrectomy with internal limiting membrane peeling intraoperatively,and nonexpansive gas tamponade after fluid-air exchange.The visual acuity and rate of macular hole closure of two groups were observed.All cases were followed up for 3-6 months.Results The closure rate of macular hole in no staining group was 97.83%(45/46),which was similar to 100.00%(38/38)in ICG staining group(x^(2)=0.836,P=0.361).In no staining group,the visual acuity at 3 months and 6 months after operation were better than that before operation(F=l.408,P=0.001).The difference of postoperative visual acuity was not statistically significant between two groups(P>0.05).Conclusion Internal limiting membrane peeling without assisted staining for treating idiopathic macular hole is an efficient and safe treatment,which avoids the toxic side effects caused by dyes and promotes postoperative visual function recovery.
