摘　　要：目的探讨腹膜后平滑肌肉瘤(LMS)的影像学表现及病理特征、肿瘤坏死与病理分级之间的关系。方法回顾性分析经手术或病理证实的39例腹膜后LMS的CT、MRI影像学特征及其病理学表现。结果39例患者的肿瘤大小2.7~20.9 cm,平均(7.08±2.9)cm,24例(61.5%)病灶位于腹膜后肾门附近或下腔静脉区,与邻近大血管关系密切;增强扫描肿瘤实性部分呈持续、渐进性强化,17例(43.6%)病灶动脉期内可见强化血管影穿行;病理分级G1级5例,G2级23例,G3级11例,不同分级肿瘤坏死程度存在差异(P<0.05),且肿瘤内坏死程度与病理分级存在正相关性(r=0.414,P=0.009<0.05)。结论腹膜后LMS较大,肿块密度及信号不均匀,病灶内坏死多见,病理学分级与坏死程度存在正相关性。Ohiective To explore the imaging features and pathological findings of retroperitoneal leiomyosarcoma(LMS),and the relationship between tumor necrosis and pathological grading.Methods The CT and MRI images of 39 cases with retroperitoneal LMS confirmed by pathology were analyzd retrospectively and compared with the pathological findings.Results The size of the.turmour was from 2.7 to 20.9 cm[mean(7.08±2.9)cm].24 cases(61.5%)were located at he retroperitonceal position near the renal portal or the inferior vena ava,closdly related to the adiacent large blood vessds.The solid part of the tumors showed continuous and progressive enhancement.The enhanced blood vessels was seen within the tumor in the arterial phase in 17 cases(43.6%).The pathological grades was G1 in 5 cases,G2 in 23 cases,and G3 in 11 cases.The proportion of tumor necrosis in dilfrent grades was dfferent(P<0.05),and the proportion of tumor necrosis was positively correlated with pathological grading(r=0.414,P=0.009<0.05).Conclusion Retroperitoneal LMS has some special imaging features,such as large sixe,heterogencity and focal necrosis.And the proportion of turmor necrosis is positively correlated with pathological grading.
