摘　　要：目的探讨前列腺MRI多参数成像联合超声引导下穿刺活检术在前列腺癌(PCa)诊断中的价值。方法选取100例前列腺特异性抗原(PSA)升高的前列腺疾病患者,其中包括2组检查方法不同的患者:A组(50例)直接行超声引导下穿刺活检术,B组(50例)首先行前列腺MRI多参数成像,后在MRI提示下行超声引导下穿刺活检术;对2组病理结果进行统计分析,比较2组检查流程的阳性率、特异度、灵敏度、假阴性率。结果B组阳性率95%、特异度86%、灵敏度72%、假阴性率5%;A组阳性率73%、特异度65%、灵敏度48%、假阴性率27%。结果显示:前列腺MRI多参数成像联合超声引导下穿刺活检术能明显提高PCa诊断阳性率,特异度和灵敏度也较直接行超声引导下穿刺活检高,同时降低了漏诊率。结论采用前列腺MRI多参数成像确定可疑病灶后,提示超声对可疑区重点取材的检查方法能够显著提高PCa诊断阳性率,有较高的特异度和灵敏度,降低漏诊率。Objective To explore the value of prostate MRI multi-parameter imaging in combination with ultrasound-guided puncture biopsy in the diagnosis of prostate cancer(PCa).Methods 100 prostate disease patients with elevated prostate-specific antigen(PSA)were ollcted,and then were divided into two grouPs with diferent exminatio methods:50 patients in group A underwent direct ultrasound-guided puncture biopsy,and other 50 patients in group B underwent tmulti-parameter MRI of prostate and then followed by ultrasound guided puncture biopsy according to the MRL The positive rate speificity,sensitivity and false negative rate of the two groups were clculated.Rsults In group B,the positive rate was 95%,the speificity 86%,the sensitivity 72%and the false negative rate 5%.In group A,positive rate was 73%,the specificity 65%,the sensitivity 48% and the false negative rate 27%.The satistial results showed that prostate MRI multi paramneter imaging in combination with ultrasound-guided puncture biopsy obviously improved the diagnostic positive rate of PCa,the specificity and sensitivity were also higher than those using single ultrasound-guided puncture biopsy,and significantly decrased the misdiagnosis.Condusion Ultrasound-guided puncture biopsy of suspicious lesions based on MRI multir paurameter imaging can sigificantly improve the diagnostic positive rate of PCa with high specificity and sensitivity。and significantly reduce the rate of misdiagnosis.
