摘 要:目的应用全视野视网膜电图(ff-ERG)评价早产儿视网膜病变(ROP)抗VEGF雷珠单抗(Ranibizumab)治疗后早期视网膜功能状态。方法选取2018年6月至2020年2月在我院行雷珠单抗玻璃体腔注射治疗的早产儿12例(24眼)作为观察组,另选取同期无眼底病变的早产儿15例(30眼)作为对照组,记录两组矫正月龄6个月左右的暗适应与明适应ERG,进行统计学比较。结果观察组ROP平均消退时间在其矫正胎龄48.75周;观察组暗适应3.0 a波平均振幅较对照组轻度降低(P<0.05);观察组明适应3.0 b波平均隐含期较对照组轻度延迟(P<0.05);其余各波的平均振幅及平均隐含期比较,差异均无统计学意义(P>0.05)。结论IVR治疗后ROP患儿的视网膜形态可恢复正常,但短期内功能稍落后于正常早产儿,患儿的长期视网膜功能发育状态仍需观察;ERG作为评价早产儿视网膜功能的手段安全可行。Objective To evaluate the early retinal function of retinopathy of prematurity after anti-VEGF ranibizumab therapy by using full field electroretinogram.Methods From June 2018 to February 2020,12 premature infants(24 eyes)who received intravitreal injection of ranibizumab in our hospital were selected as the observation group,and 15 premature infants(30 eyes)without fundus lesions were selected as the control group.The dark adaptation and light adaptation ERG were recorded in all the children at the corrected age of 6,and the results of the two groups were compared statistically.Results The average regression time of ROP in observation group was about 48.75 weeks of corrected gestational age.The average amplitude of 3.0 a wave in the observation group was slightly lower than that in the control group(P<0.05);The average latency of 3.0 b wave in the observation group was slightly delayed than that in the control group(P<0.05);There was no significant difference in the average amplitude and average latency of other waves(P>0.05).Conclusion After IVR treatment,the retinal morphology of ROP children can return to normal,but the function of ROP is slightly behind that of normal preterm infants in a short time,the long-term development of retinal function needs to be observed.ERG is a safe and feasible method to evaluate the retinal function of premature infants.
