非酒精性脂肪性肝病患者APN基因rs2241766 T/G位点多态性与肿瘤坏死因子-ɑ相关性研究

摘　　要：目的:了解深圳市非酒精性脂肪性肝病(NAFLD)患者脂联素(APN)基因rs2241766 T/G位点多态性及其水平,探讨其与肿瘤坏死因子-α(TNF-ɑ)水平之间的相关性。方法:收集2018年7月至2019年10月确诊NAFLD的患者178例为NAFLD组,选择同期健康体检者110例为对照组,采用酶联免疫吸附(ELISA)法检测两组人员血清APN基因和TNF-ɑ水平,PCR-RFLP法检测APN基因rs2241766 T/G位点多态性。结果:NAFLD组患者APN水平为(4.75±1.26)mg/L,明显低于对照组的(7.26±2.08)mg/L,而TNF-ɑ水平为(4.23±1.73)μg/L,明显高于对照组的(2.38±1.07)μg/L,差异有统计学意义(P<0.05)。重度和中度NAFLD患者APN水平分别为(2.06±0.75)mg/L和(4.32±1.04)mg/L,明显低于轻度者(5.87±1.49)mg/L,且重度低于中度;而TNF-ɑ水平分别为(6.84±2.71)μg/L和(4.62±1.98)μg/L,明显高于轻度者(3.17±1.52)μg/L,且重度高于中度,差异均有统计学意义(P<0.05)。NAFLD组患者APN基因rs2241766T/G位点TT基因型和T等位基因的检出率分别为57.30%和69.94%,明显高于对照组的25.45%和39.09%,差异有统计学意义(P<0.05)。APN基因rs2241766T/G位点TT基因型NAFLD患者APN水平为(3.97±0.92)mg/L,明显低于GT和GG基因型的(5.72±1.34)mg/L和(5.90±1.52)mg/L,差异有统计学意义(P<0.05),但GT和GG基因型之间差异无统计学意义(P>0.05)。NAFLD患者APN水平明显降低,且随病情严重程度的加重而下降,而TNF-ɑ水平明显升高,且随病情严重程度的加重而升高,经Spearman相关性分析,APN水平与TNF-ɑ呈明显负相关(P<0.05)。结论:NAFLD患者APN水平明显降低,而TNF-ɑ水平明显升高,且与病情严重程度有关。同时APN基因rs2241766T/G位点TT基因型的NAFLD患者APN水平更低,可能是深圳市NAFLD发病的危险遗传易感基因之一。Objective:To understand the polymorphism and level of rs2241766 T/G locus of adiponectin(APN)gene in patients with nonalcoholic fatty liver disease(NAFLD)in Shenzhen,and to explore the correlation between the tumor necrosis factor-α(TNF-α)level.Methods:From July 2018 to October 2019,178 patients who came to the hospital for physical examination and were diagnosed with NAFLD were collected as NAFLD group,During the same period,110 non-NAFLD patients were selected as the control group,using enzyme-linked immunosorbent(ELISA)method to detect the levels of APN and TNF-α,at the same time,the rs2241766 T/G polymorphism of APN gene was detected by the polymerase chain reaction restriction fragment length polymorphism(PCR-RFLP)method.Results:The level of APN in patients with NAFLD group was(4.75±1.26)mg/L,significantly lower than(7.26±2.08)mg/L in the control group,and the level of TNF-αwas(4.23±1.73)μg/L,significantly higher than(2.38±1.07)μg/L in the control group,the difference was statistically significant(P<0.05).The levels of APN in patients with severe and moderate NAFLD were(2.06±0.75)mg/L and(4.32±1.04)mg/L,significantly lower than(5.87±1.49)mg/L in the mild,and severe lower than moderate,and the level of TNF-αin severe and moderate NAFLD patients were(6.84±2.71)μg/L and(4.62±1.98)μg/L,significantly higher than(3.17±1.52)μg/L in mild,and severe higher than moderate,the difference was statistically significant(P<0.05).The detection rates of TT genotype and T allele at rs2241766T/G site of APN gene in NAFLD group were 57.30%and 69.94%,respectively,which were significantly higher than 25.45%and 39.09%in the control group,the difference was statistically significant(P<0.05).The level of APN in TT genotype NAFLD patients with APN gene rs2241766T/G site was 3.97±0.92 mg/L,significantly lower than 5.72±1.34 mg/L and 5.90±1.52 mg/L of GT and GG genotypes,the difference was statistically significant(P<0.05),but there was no statistically significant difference between GT and GG genotypes(P>0.05).Th
