深部热疗在卵巢癌合并恶性腹腔积液复发患者中的临床疗效

摘　　要：目的观察深部热疗联合顺铂腹腔灌注化疗+紫杉醇静脉化疗在晚期卵巢癌合并腹腔积液复发患者中的治疗效果。方法选取本科室卵巢癌合并恶性腹腔积液复发的患者18例,随机分为实验组和对照组。实验组患者接受顺铂腹腔灌注化疗+紫杉醇静脉化疗联合深部热疗,对照组患者接受顺铂腹腔灌注化疗+紫杉醇静脉化疗。观察并比较两组患者的临床治疗效果、不良反应及血清人附睾蛋白4(HE4)、糖类抗原125(CA125)水平的变化。结果实验组患者有效率高于对照组,差异有统计学意义(P<0.05);两组患者均未发生严重不良反应,且不良反应的发生差异无统计学意义(P>0.05);治疗后,两组患者血HE4、CA125水平低于治疗前,且实验组表达水平更低,差异有统计学意义(P<0.05)。结论深部热疗联合顺铂腹腔灌注化疗+紫杉醇静脉化疗治疗卵巢癌合并恶性腹腔积液复发患者的疗效较好。深部热疗可有效控制、减少腹腔积液,并降低HE4、CA125的水平,且不良反应不显著。Objective To observe the therapeutic effect of deep hyperthermia combined with intraperitoneal chemotherapy with cisplatin and intravenous chemotherapy with paclitaxel in patients with advanced ovarian cancer complicated with recurrent ascites.Methods Eighteen cases with ovarian cancer complicated with recurrence of malignant ascites were randomly divided into experimental group and control group.The experimental group received cisplatin intraperitoneal perfusion chemotherapy+paclitaxel intravenous chemotherapy combined with deep hyperthermia,while the control group received cisplatin intraperitoneal perfusion chemotherapy+paclitaxel intravenous chemotherapy.The clinical efficacy,adverse reactions and changes of serum levels of human epididymal protein 4 and carbohydrate antigen 125 were observed and compared between the two groups.Results The effective rate of the experimental group was higher than that of the control group,and the difference was statistically significant(P<0.05);No serious adverse reactions occurred in the two groups,and there was no difference in the occurrence of adverse reactions(P>0.05);After treatment,the levels of HE4 and CA125 in the two groups were lower than those before treatment,and the expression level of experimental group was lower,the difference was statistically significant(P<0.05).Conclusion Deep hyperthermia combined with cisplatin intraperitoneal perfusion chemotherapy plus paclitaxel intravenous chemotherapy in the treatment of ovarian cancer patients with malignant ascites recurrence is better,can effectively control and reduce the ascites,and reduce the level of HE4,CA125,and the adverse reactions are not obvious.
