SMILE近视矫正手术的疗效和安全性观察

摘　　要：目的:评估飞秒激光小切口角膜基质透镜取出术(SMILE)进行近视矫正的有效性、可预测性和安全性。方法:采用病例回顾研究,纳入2020年1月~8月在中日友好医院行SMILE近视矫正手术患者102例,共199眼。评估术前、术后1周、1个月、3个月及6个月时裸眼视力(UCVA)、最佳矫正视力(BCVA)和屈光度情况,观察术中及术后相关并发症、术后不适症状。结果:观察终点UCVA达到或超过术前BCVA有194眼(97.5%)。有效性指数为1.103±0.109。术后观察终点BCVA(LogMAR-0.041±0.045)较术前(Log MAR 0±0.016)显著提高(F=3.271,P=0.015)。安全性指数为1.110±0.100。手术设计矫正等效球镜度(SE)与观察终点实际矫正SE具有高度相关性(R=0.953,P=0.000)。未发生影响视力的严重并发症。结论:SMILE近视矫正手术具有良好的有效性、可预测性及安全性,术后可达到稳定、良好的视力。Objective:To investigate the efficacy,predictability and safety of small incision lenticule extraction(SMILE)for myopia correction.Methods:A retrospective study was performed on 102 patients(199 eyes)with myopia who underwent SMILE in China-Japan Friendship Hospital from January 2020 to August 2020.The uncorrected visual acuity(UCVA),best corrected visual acuity(BCVA)and refractive error were measured preoperatively and postoperatively at 1 week,1 month,3 months and 6 months follow up.The operative and postoperative complications were observed.We also collected the patients’ complaint after the surgery.The results were analyzed with repeated measurement analysis of variance.Results:At the end of the observation,194 eyes(97.5%)had an equal or better UCVA than their preoperative BCVA.The efficacy index was 1.103 ±0.109.The BCVA(LogMAR)at the end of the observation was significantly increased than that before the surgery(-0.041±0.045 vs.0±0.016,F=3.271,P=0.015).The safety index was 1.110±0.100.The attempted spherical equivalent corrected was positively correlated with achieved SE(R=0.953,P=0.000).No severe complications that cause BCVA influence were observed.Conclusion:SMILE has fabourable performance on effectiveness,predictability and safety for myopia correction,and a stable and satisfied visual acuity is achieved after the operation.
