摘　　要：目的:探讨乳腺癌患者外周血循环肿瘤细胞(CTC)检测阳性率与原发肿瘤临床病理特征及预后的关系。方法:前瞻性收集41例乳腺癌患者术前外周血,密度梯度离心进行CTC富集,甲苯胺蓝染色,CK联合上皮黏附分子免疫细胞化学法鉴定CTC。采用χ2检验分析CTC阳性率与临床病理特征及预后的关系。结果:乳腺癌外周血CTC阳性率与分子分型、原发肿瘤T分期及TNM分期具有明显相关性(均P<0.05)。与年龄、激素受体、Ki67、Her-2表达、淋巴结、远处转移无显著相关性(均P>0.05)。平均随访75.9±2.59个月,CTC阳性组与CTC阴性组在远处转移及死亡发生率方面均无显著性差异(P=0.555)。结论:乳腺癌外周血CTC的检测具有显著临床意义。Objective:To investigate the relationship between the detection of circulating tumor cells(CTCs)and clinicopathological features of breast cancer patients as well as their prognosis.Methods:Preoperative peripheral blood samples of 41 patients suffering from breast cancer were collected prospectively.CTCs were enriched by density gradient centrifugation, stained with toluidine blue,and identified by CK combining with EpCAM immunocytochemistry.The relationship between the positive rate of CTCs and clinicopathological features of the patients and their prognosis were analyzed by chi square test.Results:The positive rate of CTCs in peripheral blood circulation of breast cancer patients was significantly correlated with their molecular classifications,primary tumor T staging and TNM subgroup staging(P<0.05).There was no significant correlation with age,hormone receptor status,Ki-67 level,HER-2 expression,lymph node and distant metastasis(P>0.05).The average follow-up was 75.9±2.59 months.There was no significant difference in the incidence of distant metastasis and death between CTC positive group and CTC negative group(P=0.555).Conclusion:The detection of CTCs in peripheral blood circulation of breast cancer patients has significant clinical significance.
