摘 要:目的探讨转化生长因子β_(1)(TGF-β_(1))和p53蛋白在弥漫性脑胶质瘤中的表达及意义。方法收集首都医科大学附属北京天坛医院2019年3月至12月各级星形细胞瘤及少突胶质细胞瘤患者57例。采用免疫组织化学法检测TGF-β_(1)蛋白和p53蛋白的表达情况,分析二者在不同类型弥漫性脑胶质瘤中的表达特点。结果57例患者中TGF-β_(1)阳性表达率为49.1%(28/57),p53阳性表达率为45.6%(26/57)。星形细胞瘤(IDH突变型)p53阳性表达率(54.3%,19/35)高于少突胶质细胞瘤(0,0/7),差异有统计学意义(P=0.011)。表达TGF-β_(1)的弥漫性脑胶质瘤较多累及大脑颞叶(56.3%,9/16)、顶叶(83.3%,5/6),而较少累及额叶(42.4%,14/33)、枕叶(0,0/2)(P=0.142)。结论p53蛋白阳性表达是各级星形细胞瘤(IDH突变型)重要的标志物之一,可以辅助用于星形细胞瘤(IDH突变型)和少突胶质细胞瘤的鉴别诊断。表达TGF-β_(1)的弥漫性脑胶质瘤有累及大脑颞叶、顶叶的趋势。Objective To investigate the expressions of transforming growth factorβ_(1)(TGF-β_(1))and p53 proteins in diffuse glioma and its significance.Methods A total of 57 patients with astrocytoma and oligodendroglioma of various levels were collected from Beijing Tiantan Hospital,Capital Medical University between March and December in 2019.Immunohistochemical staining was used to detect the expressions of TGF-β_(1)protein and p53 protein.The expression characteristics of different types of diffuse gliomas were also analyzed.Results The positive expression rate of TGF-β_(1)in 57 patients was 49.1%(28/57),and the positive expression rate of p53 was 45.6%(26/57).The positive expression rate of p53 in astrocytoma(IDH mutant type)was higher than that in oligodendroglioma[54.3%(19/35)vs.0(0/7),P=0.011].Diffuse gliomas expressing TGF-β_(1)mostly involved the temporal lobe(56.3%,9/16)and parietal lobe(83.3%,5/6)of the brain more,but less involved the frontal lobe(42.4%,14/33)and occipital lobe(0,0/2)(P=0.142).Conclusion Positive expression of p53 protein is an important marker of astrocytoma at all grades(IDH mutant type),which can be used for the differential diagnosis of astrocytoma(IDH mutant type)and oligodendroglioma.Diffuse gliomas expressing TGF-β_(1)tend to involve the temporal lobe and parietal lobe of the brain.
