摘 要:目的:探讨血清微小核糖核酸(miRNA)的表达水平对急性脑出血患者神经功能重建的指导价值。方法:选取2018-03—2019-06收治的急性脑出血患者188例为研究对象,根据患者治疗后的神经功能情况进行分组,其中神经功能基本正常的患者归纳于成功组(96例),神经功能存在轻度缺损的56例,中度缺损24例,重度缺损12例,并将其归纳于失败组(92例)。观察2组治疗前后的血清miRNA(miRNA-21、miRNA-135)的表达水平,并分析不同程度神经功能缺损患者的miRNA-21、miRNA-135相对表达量。应用受试者工作特征(ROC)曲线分析血清miRNA-21、miRNA-135相对表达量在预测急性脑出血患者神经功能重建的效能。结果:成功组与失败组治疗前的miRNA-21、miRNA-135相对表达量比较,差异无统计学意义(P>0.05);成功组治疗后的miRNA-21、miRNA-135相对表达量均高于失败组,差异有统计学意义(P<0.05);轻度、中度及重度的miRNA-21、miRNA-135相对表达量呈依次降低,差异有统计学意义(P<0.05);血清miRNA-135、miRNA-21水平在预测急性脑出血患者神经功能重建的曲线下面积(AUC)分别为:0.837、0.656,以最大约登指数计算出miRNA-135及miRNA-21最大AUC面积相应参数截断值,其中miRNA-135截断值为0.535(敏感度83.61%,特异性91.29%),miRNA-21截断值为0.234(敏感度75.31%,特异性69.22%)。结论:血清miRNA-135、miRNA-21的表达水平与急性脑出血患者神经功能重建程度有关。在临床管理中,检测急性脑出血患者miRNA-135、miRNA-21表达水平,对急性脑出血患者神经功能重建进展的预测具有一定价值。Objective:To explore the value of serum microRNAs expression in guiding neurological function reconstruction in patients with acute cerebral hemorrhage. Method:A total of 188 patients with acute cerebral hemorrhage admitted to our hospital from March 2018 to June 2019 were selected as the research objects.The patients with normal nerve function were divided into success group(96 cases),56 patients with mild neurological deficits,24 patients with moderate neurological deficits and 12 patients with severe neurological deficits.The patients were divided into failure group(92 cases).The serum levels of microRNAs(microRNAs-21,microRNAs-135)were observed before and after treatment in both groups,and the relative expressions of microRNAs-21 and microRNAs-135 in patients with different degrees of neurological deficits were analyzed.The ROC curve was used to analyze the relative expression of serum microRNA-21 and microRNA-135 in predicting the effect of neurological function reconstruction in the patients with acute cerebral hemorrhage. Result:There was no significant difference in the relative expression of microRNA-21 and microRNA-135 between the successful group and the failure group before treatment(P>0.05);the relative expression of microRNA-21 and microRNA-135 in the successful group after treatment was higher than those in the failure group(P<0.05);the relative expression of microRNA-21 and microRNA-135 in the mild,moderate and severe groups was lower than those in the failure group(P<0.05);the relative expression of microRNA-21 and microRNA-135 in the mild,moderate and severe groups was significantly lower(P<0.05).Serum levels of microRNA-135 and microRNA-21 were used to predict the AUC area of neurological function reconstruction in the patients with acute cerebral hemorrhage:microRNA-135(0.837)and microRNA-21(0.656).The cut-off values of the corresponding parameters of the maximum AUC area of microRNA-135 and microRNA-21 were calculated with the approximate landing index.The cut-off values of microRNA-135 and m
