摘 要:目的:掌握cisAB血型的血清学反应格局,结合分子生物学方法,正确进行ABO血型鉴定,保证临床输血安全。方法:采用微柱凝胶、吸收放散、唾液血型物质鉴定等方法对1例正反不符样本进行血型血清学鉴定;并进一步采用序列特异性引物聚合酶链反应法(PCR-SSP)和DNA测序分型法对先证者进行血型基因分型。结果:血型血清学鉴定该样本为A2Bx,血清中存在抗-B抗体;PCR-SSP基因分型结果为B/O01型,测序分型结果为ABO^(*)cisAB.01/O.01.01。结论:对于ABO正反定型不符的样本,基因分型技术应用有助于解决疑难血型问题,为疑难血型的精准输血奠定基础。Objective:To grasp the serum blood reaction pattern of Cis AB blood group and explore its genotyping and molecular mechanism,and correct blood group identification to ensure the safety of clinical blood transfusion. Method:ABO blood group was identified by serological detection,including microcolumn gel and forward and reverse group methods.Exon 6 and 7 of the ABO genes were screened by PCR sequence specific primer(PCR-SSP)method and directly sequencing. Result:The direct typing of blood group serology was AB weak,anti-B antibody was detected in the reverse group;while the genotype of ABO in human erythrocyte was B/O01,and the result of gene sequencing was ABO^(*)cisAB.01/O.01.01.Conclusion:The application of genotyping and gene sequencing technology may be helpful for the identification of difficult blood groups for samples with inconsistent forward and reverse ABO typing.The combination of serum blood method and molecular biology method can lay a foundation for the identification of difficult blood groups and precise blood transfusion.
