EZH2、IMP3、Glut1蛋白在恶性间皮瘤组织中的表达及意义

摘　　要：目的研究zeste基因增强子人类同源物2(EZH2)、胰岛素样生长因子ⅡmRNA结合蛋白3(IMP3)、葡萄糖转运蛋白1(Glut1)在恶性间皮瘤(MM)组织中的表达及临床意义。方法以50例MM患者为观察组,20例反应性间皮细胞增生(RMH)患者为对照组,应用免疫组织化学SP法检测两种组织中EZH2、IMP3、Glut1蛋白表达,分析在良恶性间皮组织中表达的差异,并分析MM中3种抗体的表达及与临床病理参数的关系。结果两种组织中3种抗体的表达率比较,差异有显著性(χ2=15.36~35.19,P<0.05)。EZH2、IMP3、Glut1对MM诊断的灵敏度分别为88%、60%和68%,特异度分别为90%、95%和100%;EZH2并IMP3、EZH2并Glut1、IMP3并Glut1、EZH2并IMP3并Glut1对MM诊断的灵敏度分别为90%、98%、84%、98%。IMP3在MM腹膜组及胸膜组中阳性表达率比较,差异有显著性(χ2=5.22,P<0.05);IMP3的表达与MM患者其他临床病理学特征无关(P>0.05);EZH2、Glut1的表达与MM临床病理学特征无关(P>0.05)。IMP3阳性表达的MM患者生存率显著低于阴性表达者,差异有显著性(χ2=10.35,P<0.05)。结论EZH2、IMP3、Glut1联合检测对提高MM和RMH的鉴别诊断有重要临床价值。IMP3是影响MM患者预后的风险因素。ObjectiveTo investigate the expression and clinical significance of enhancer of zeste homolog 2(EZH2),insulin-like growth factorⅡmRNA-binding protein 3(IMP3),and glucose transporter 1(Glut1)in malignant mesothelioma(MM)tissue. Methods A total of 50 MM patients were enrolled in the observation group and 20 reactive mesothelial hyperplasia(RMH)patients were enrolled in the control group.The immunohistochemical SP method was used to determine the expression of EZH2,IMP3,and Glut1 proteins in both tissues and analyze the difference in their expression between benign and malignant mesothelial tissues.In addition,an analysis was performed in terms of the expression of the three antibodies in MM and the relationship of them with clinicopathological parameters. Results There were significant differences in the expression of the three antibodies between the two tissues(χ2=15.36-35.19,P<0.05).The sensitivity of EZH2,IMP3,and Glut1 was 88%,60%,and 68%,respectively,for MM diagnosis,and the specificity was 90%,95%,and 100%,respectively.When EZH2,IMP3,and Glut1 were used in combination,the sensitivity for MM diagnosis was 90%(EZH2/IMP3),98%(EZH2/Glut1),84%(IMP3/Glut1),and 98%(EZH2/IMP3/Glut1).There was a significant difference in the positive expression of IMP3 between MM peritoneal group and pleural group(χ2=5.22,P<0.05).However,the IMP3 expression was not correlated with other clinicopathological features(P>0.05).The EZH2 and Glut1 expression was not correlated with any MM clinicopathological parameter(P>0.05).The survival rate of MM patients with IMP3 positive expression was significantly lower than that of MM patients with negative expression(χ2=10.35,P<0.05). Conclusion The combined measurement of EZH2,IMP3,and Glut1 has important clinical value in improving differential diagnosis of MM and RMH.IMP3 is an independent risk factor affecting the prognosis of MM.
