摘 要:目的以新西兰兔为动物实验模型,探讨应用磁吻合技术实现肠造瘘口还纳术的可行性及安全性。方法选取体质量相近的新西兰种兔15只,建立结肠造瘘口模型,均选用一对环状磁铁作为磁吻合的装置,完成还纳手术操作,记录还纳术操作时间;观察动物存活情况及并发症发生情况;获取吻合口肠段,测得爆破压力,并观察吻合口外观和组织病理学形态。结果实施磁吻合还纳术平均操作时间为(16.80±2.74)min,体内磁环排出所需的平均时间为(6.11±0.99)d,实验动物存活率为80%,存活的兔进食、排便正常。解剖后测得吻合肠管平均爆破压力为(18.86±0.93)kPa,肠吻合口外观光滑平整,组织学形态显示吻合口处完全愈合。结论利用磁吻合技术完成兔的结肠造瘘口还纳术操作简单,具有一定的可行性,具备微创技术临床应用潜力。Objective To investigate the feasibility and safety of magnetic compression anastomosis in stoma closure operation with New Zealand rabbits as the model for animal experiment.Methods A total of 15 New Zealand rabbits with si-milar body weight were selected to establish a model of colostomy,and a pair of magnetic rings were selected as the device for magnetic compression anastomosis to complete stoma closure operation.The time of stoma closure operation was recorded,and the survival and complications of the animals were observed.The intestinal segment for anastomosis was obtained,the bursting pressure was measured,and the appearance and histopathology of the anastomosis were observed.Results The mean time of stoma closure operation was(16.80±2.74)min,and the mean time for the removal of magnetic rings was(6.11±0.99)d.The survival rate of the experimental animals was 80%,and the rabbits that survived could eat and defecate normally.The mean bursting pressure of the anastomosed intestine was(18.86±0.93)kPa after anatomy.The intestinal anastomosis was smooth and flat,and histomorphology showed that the anastomosis was completely healed.Conclusion Magnetic compression anastomosis for stoma closure operation has simple operation and certain feasibility,and therefore,it has the potential of clinical application as a minimally invasive technique.
