阿立哌唑与氯氮平对慢性精神分裂症患者疗效及脂质指标的影响比较

摘　　要：目的分析阿立哌唑与氯氮平对慢性精神分裂症患者的治疗效果及脂质指标的影响。方法80例慢性精神分裂症患者,随机分为阿立哌唑组和氯氮平组,每组40例。阿立哌唑组患者给予口服阿立哌唑治疗,氯氮平组患者给予口服氯氮平治疗。比较两组患者的临床疗效、不良反应发生率及治疗前后的脂代谢指标水平。结果阿立哌唑组患者的临床治疗总有效率为95.00%,高于氯氮平组的80.00%,差异有统计学意义(P<0.05)。阿立哌唑组患者治疗前后的甘油三酯(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)水平组内比较,差异无统计学意义(P>0.05);氯氮平组患者治疗前后的TC、LDL-C、HDL-C水平组内比较,差异无统计学意义(P>0.05);氯氮平组治疗后的TG高于治疗前,且高于阿立哌唑组,差异有统计学意义(P<0.05)。阿立哌唑组患者的不良反应发生率为7.50%,低于对照组的27.50%,差异有统计学意义(P<0.05)。结论氯氮平对慢性精神分裂症患者的脂代谢指标有明显影响,但阿立哌唑对脂代谢指标无明显影响,且相比氯氮平疗效更加显著,不良反应更少,临床安全性更好,临床可将其作为慢性精神分裂症患者的首选药物进行应用。Objective To analyze the efficacy and lipid index between aripiprazole and clozapine in patients with chronic schizophrenia.Methods A total of 80 patients with chronic schizophrenia were randomly divided into aripiprazole group and clozapine group,with 40 cases in each group.Aripiprazole group was treated with oral aripiprazole,and clozapine group was treated with oral clozapine.The clinical efficacy,incidence of adverse reactions and lipid metabolism index levels before and after treatment were compared between the two groups.Results The total effective rate of clinical treatment of aripiprazole group was 95.00%,which was higher than 80.00%of clozapine group,and the difference was statistically significant(P<0.05).In aripiprazole group,there was no statistically significant difference in triglycerides(TG),total cholesterol(TC),low density lipoprotein cholesterol(LDL-C),high density lipoprotein cholesterol(HDL-C)before and after treatment(P>0.05).In clozapine group,there was no statistically significant difference in TC,LDL-C and HDL-C before and after treatment(P>0.05).After treatment,the TG of clozapine group was higher than that before treatment and aripiprazole group,and the difference was statistically significant(P<0.05).The incidence of adverse reactions of aripiprazole group was 7.50%,which was lower than 27.50%of the control group,and the difference was statistically significant(P<0.05).Conclusion Clozapine has a significant effect on lipid metabolism indicators in patients with chronic schizophrenia,but aripiprazole has no significant effect on lipid metabolism indicators.Besides,compared with clozapine,it has more significant efficacy with fewer adverse reactions and better clinical safety,so clozapine can be used clinically as the drug of choice for patients with chronic schizophrenia.
