摘  要:目的:探讨16排螺旋CT在伴甲状腺炎的甲状腺良恶性结节中的诊断价值。方法:回顾性分析我院2015年1月-2019年10月经手术证实为桥本氏甲状腺炎伴结节56例患者的CT图像。结果:56例患者中,经手术证实恶性结节21例(37.5%),良性结节35例(62.5%)。良性结节中,结节性甲状腺肿16例,甲状腺腺瘤5例,桥本氏结节14例;恶性结节中,21例患者全诊断为甲状腺乳头状癌,其中14例为微小乳头状癌(≤lcm)。良性结节大部分表现为囊实性结节,边界清晰,伴结节状或弧形钙化(多位于病灶边缘),增强后病灶边界亦清晰;恶性结节表现为低密度实性结节,边界多不清晰,部分可见微钙化(<2mm),增强后结节边界较平扫模糊。结论:16排螺旋CT在伴甲状腺炎的甲状腺良恶性结节中具有较大的应用价值。Objective:To investigate the diagnostic value of 16-slice spiral CT in benign and malignant thyroid nodules with thyroiditis.Methods:Retrospective analysis of the CT images of 56 patients with Hashimoto’s thyroiditis with nodules confirmed by surgery from January 2015 to October 2019 in our hospital.Results:Among 56 patients,21 cases(37.5%)had malignant nodules and 35 cases(62.5%)had benign nodules.Among the benign nodules,16 cases had nodular goiter,5 cases had thyroid adenoma,and 14 had Hashimoto’s nodules.Among the malignant nodules,21 patients were all diagnosed with papillary thyroid carcinoma,14 of which were micropapillary Cancer(≤lcm).Most of the benign nodules are cystic nodules,with clear boundaries,with nodular or arcal calcification(mostly at the edge of the lesion),and the boundary of the lesions after the enhancement is also clear;malignant nodules appear as low-density solid nodules,Most of the borders are not clear,and microcalcification(<2 mm)can be seen in some parts.The borders of nodules are more blurred after the enhancement.Conclusion:16-slice spiral CT has great application value in benign and malignant thyroid nodules with thyroiditis.
