面部皮肤鳞状细胞癌扩大切除术联合全厚植皮及放疗的临床效果观察

摘　　要：目的分析观察面部皮肤鳞状细胞癌扩大切除术联合全厚植皮及放疗的临床效果。方法本文所选对象为我院2014年3月至2020年3月收治的50例面部皮肤鳞状细胞癌患者,全部患者均接受扩大切除术联合全厚植皮及放疗,观察分析临床疗效。结果全部50例患者的肿瘤组织均在术中得到完整切除,病理检查结果发现底部和切缘均没有残留肿瘤组织,鳞癌分化程度为:31例患者为高分化,15例患者为中分化,4例患者为低分化。术后皮片坏死面积为:46例患者<5.0%,3例患者为5%-10%,1例患者>10%-20%。术后对患者进行为期1年时间的随访发现,2例患者出现肿瘤局部复发,复发率为4.0%;复发患者后续接受再次放疗或手术治疗。全部50例患者中,美容效果为:32例患者为优,16例患者为良,2例患者为差。结论面部皮肤鳞状细胞癌患者选择扩大切除术联合全厚植皮及放疗能取得显著的临床效果,具有较高的术后皮片存活率,外观效果和美容修复理想,肿瘤复发转移风险低,值得临床推广。Objective To analyze and observe the clinical effect of extended resection combined with full thickness skin grafting and radiotherapy for facial squamous cell carcinoma.Methods 50 cases of facial skin squamous cell carcinoma were selected in our hospital from March 2014 to March 2020.All patients received extended resection combined with full thickness skin grafting and radiotherapy,and the clinical efficacy was observed and analyzed.Results The tumor tissues of all 50 patients were completely removed during the operation.Pathological examination showed that there was no residual tumor tissue at the bottom and cutting edge.The degree of differentiation of squamous cell carcinoma was as follows:31 patients were well differentiated,15 patients were moderately differentiated,and 4 patients were poorly differentiated.The area of skin graft necrosis was less than 5.0%in 46 patients,5%-10%in 3 patients and 10%-20%in 1 patient.After 1-year follow-up,2 patients had local tumor recurrence,the recurrence rate was 4.0%;the recurrence patients received radiotherapy or surgery again.In all 50 patients,the cosmetic effect was excellent in 32 patients,good in 16 patients and poor in 2 patients.Conclusion Patients with facial skin squamous cell carcinoma choose extended resection combined with full-thickness skin grafting and radiotherapy can achieve significant clinical effect,with high postoperative skin graft survival rate,ideal appearance effect and cosmetic repair,low risk of tumor recurrence and metastasis,which is worthy of clinical promotion.
