即刻种植美学修复治疗对前牙外伤患者临床效果及整体美观度的影响

摘　　要：目的:探讨即刻种植美学修复治疗对前牙外伤患者临床效果及整体美观度的影响。方法:选取2016年2月-2019年12月笔者科室接诊的31例前牙外伤患者,根据治疗不同分为两组,对照组采用延期种植美学修复治疗,研究组采用即刻种植美学修复治疗。比较两组附着高度、整体美观、色泽及咀嚼功能,比较两组不同时点美学效果、牙周袋深度,两组的种植成功率、并发症发生率及牙槽嵴宽度变化量。结果:研究组附着高度、整体美观、色泽、咀嚼功能评分均高于对照组(P<0.05);研究组术后30d、术后3个月、术后半年、术后1年的美学效果均优于对照组(P<0.05);研究组术后30d、术后3个月、术后半年、术后1年的牙周袋深度均低于对照组(P<0.05);两组种植成功率、并发症发生率比较无明显差异(P>0.05);种植体肩台处、种植体肩台下2mm、4mm、6mm测量位点牙槽嵴宽度变化量两组对比无显著性差异(P>0.05),研究组种植体肩台下8mm位点牙槽嵴宽度变化量小于对照组(P<0.05)。结论:即刻种植美学修复治疗前牙外伤患者,患者主观满意度较高,具有更好的美学效果,患者牙周组织健康程度能得到更明显改善,安全性、成功率也较高,具有早期恢复功能、美观及治疗周期短等优点,值得临床推广应用。Objective To investigate the effect of immediate implant aesthetic restoration on the clinical effect and overall aesthetics of patients with anterior teeth injury.Methods 31 patients with anterior teeth trauma from February 2016 to December 2019 were selected and divided into two groups according to different treatments.The control group was treated with delayed implant aesthetic restoration,and the study group was treated with immediate implant aesthetic restoration.The attachment height,overall appearance,color,masticatory function,aesthetic effect,periodontal pocket depth,implantation success rate,complication rate and alveolar ridge width of the two groups were compared.Results The scores of attachment height,overall appearance,color,and chewing function in the study group were higher than those in the control group(P<0.05),the aesthetic effects of the study group were better than those of the control group at 30 days,3 months,half a year,and 1 year.The control group(P<0.05).The depth of periodontal pockets in the study group 30 days after surgery,3 months after surgery,half a year after surgery,and 1 year after surgery were lower than those of the control group(P<0.05).There was no significant difference in the incidence of complications(P>0.05),there was no significant difference between the two groups in the changes in the width of the alveolar ridge at the implant shoulder and 2mm,4mm,and 6mm below the implant shoulder(P>0.05).The change in the width of the alveolar ridge at 8mm under the implant shoulder in the study group was smaller than that in the control group(P<0.05).Conclusion Immediate implant aesthetic restoration in the treatment of anterior teeth trauma patients,patients with higher subjective satisfaction,better aesthetic effect,patients with periodontal tissue health can be more significantly improved,safety,success rate is also higher,with early recovery function,early recovery beauty,short treatment cycle and other advantages,clinical promotion value is higher.
