达原饮加减治疗术后、感染治愈后功能性低热32例临床观察

摘　　要：目的:观察达原饮加减治疗术后、感染治愈后功能性低热的效果。方法:收集河南中医药大学第三附属医院2015年6月-2019年4月门诊及住院患者术后、感染治愈后低热64例,随机分为治疗组与对照组各32例。对照组口服柴胡口服液、生脉饮口服液,每次各1支,每日3次;治疗组以达原饮加减,达原饮方原方组成:厚朴12 g,槟榔12 g,草果12 g,知母10 g,白芍10 g,黄芩10 g,甘草6 g,每日1剂,水煎400 mL,分2次温服。加减运用:气虚者加补中益气汤,阴虚者二至丸,血虚加四物汤合当归补血汤,阳虚加四逆汤,失眠者加交泰丸,自汗者加玉屏风散,纳差者加焦三仙各15 g,大便不通者加生白术12 g,全瓜蒌12 g,大便溏泄者加生山楂30 g。上药加水800 mL,武火烧开后改文火煎煮15~20 min,取汁400 mL,分2次温服,每日1剂,7 d为1个疗程,总疗程14 d。以2周为观察周期,对患者的临床表现和体温波动情况进行观察记录。结果:治疗组痊愈24例,显效5例,有效1例,无效2例,总有效率为93.8%(30/32);对照组痊愈16例,显效8例,有效2例,无效6例,总有效率为81.3%(26/32)。结论:治疗组疗效显著优于对照组,达原饮加减治疗术后、感染治愈后功能性低热的效果明显,同时表明达原饮加减能起到开达膜原,清除体内邪热,调节人体阴阳平衡,使功能性低热向愈的功效,且安全无不良反应,值得推广。Objective:To observe the effect of Modified Dayuan decoction(达原饮加减)in treating 32 cases of functional low fever after operation and infection cure.Methods:64 patients with fever and after operation and infection cure were collected from June 2015 to April 2019.They were randomly divided into the treatment group and the control group.The control group received oral Chaihu oral liquid(柴胡口服液)and Shengmaiyin oral liquid(生脉饮口服液),1/time,3 times a day;the treatment group was treated with Modified Dayuan decoction,and the original composition was:Magnolia officinalis 12 g,Areca areca 12 g,acanthopanax 12 g,radix paeoniae 10 g,Radix Scutellariae 10 g,licorice 6g,one dose a day and the decoction was 400 mL,which was divided into 2 times and taken when it was warm.Modified application:For patients with deficiency of Qi,Buzhong Yiqi decoction(补中益气汤)was added,Erzhi pill(二至丸)was added for patients with deficiency of Yin,Siwu decoction(四物汤)combined with Dangui Buxue decoction(当归补血汤)was added for patients with deficiency of blood and Sini decoction(四逆汤)wash added for patients with deficiency of blood,Jiaotai pill(交泰丸)was added for patients with insomnia,Yupingfeng powder(玉屏风散)was added for patients with self-perspiration,Jiaosanxian(焦三仙)15 g respectively was added for patients with poor appetite;atractylodes 12 g,total trichosanthis 12 g were added for patients with constipation and 30 g of raw hawthorn was added for patients with loose stool.Add 800 mL water to the medicine,boil it over a moderate fire for 15-20 min,take 400 mL and take it in two warm doses,one dose per day,7 days was for one course of treatment,and the total course of treatment was 14 days.The clinical manifestations and temperature fluctuations of the patients were observed and recorded in a 2-week observation period.Results:In the treatment group,24 cases were cured,5 cases were markedly effective,1 case was effective,2 cases were ineffective,and the total effective rat
